Dr. Cristina Piras presents the current status of the project Better4U, a 4-year Horizon Europe funded project (2023-2027) that aims to develop comprehensive research and innovative interventions to address and reverse the widespread increase in and weight gain. https://better4u.eu.

At the 21th International Workshop on Neonatology and Pediatrics, Cagliari, T-Hotel, 22-25 October 2024, Dr. Cristina Piras, researcher at the University of Cagliari (UNICA), presented the role of UNICA's Metabolomics Laboratory in the project, which comprises primarily two main tasks:

I) The study of the Interplay between psychosocial, biological, and behavioural factors, as obesity and weight change are not solely driven by risk (e.g., , ), but are also strongly modulated by and contextual factors. Preliminary evidence, including transcript discussions, suggests that s*x and gender play specific roles in pathways to weight gain and barriers to weight loss. UNICA is already performing a meta-analysis of data from targeted proton nuclear magnetic resonance (^1H-NMR) spectra, using datasets generated employing the Nightingale Health, Metaboneer, and Biocrates platforms from an extensive multi-cohort study. The aims are to (i) evaluate the association between comprehensive metabolic profiles and continuous body mass index ( ) across diverse European cohorts and (ii) assess the influence of geographical clustering on these associations. This work aligns with the broader objective to develop a refined lifestyle risk score that captures not only diet and physical activity but also upstream determinants.

II) A Multicentre clinical trial focused on implementation, monitoring, and evaluation.
This task will design and deliver a multicentre, open-label, parallel-group randomised controlled trial across seven sites. The arms are the control group, which is based on standard counselling by / , and the intervention group, in which personalized counseling will be performed via the BETTER4U platform, integrating AI-based causal models and polygenic risk scores. The primary outcome will be to observe a change in BMI after 6 months. Whereas, the secondary outcome will be a reduction of ≥5% weight loss at 6 months and maintenance at 12 months; changes in biomarkers (metabolomics (UNICA team), lipidomics, adipokines, cardiometabolic and inflammatory markers, gut microbiota); blood pressure; lifestyle parameters; quality of life; psychosocial measures (anxiety, depression, motivation); and BMI changes in offspring/family members. The sample size is approximately 1,022 participants (~146 per site), with balanced recruitment by s*x, and in particular, the UNICA team will oversee biospecimen handling and perform NMR-based metabolomics analyses at two time points (baseline and 6-month follow-up). The team will also contribute to real-time monitoring of participants and processes, and evaluation of effectiveness and adherence/feasibility to inform iterative improvements to the programme.

Il progetto Life MILCH, finanziato dall' Unione Europea (LIFE 18 ENV/IT/000460), vuole promuovere il benessere e la salute del bambino, migliorando la qualità dell’ambiente che circonda la coppia madre-infante, in modo da ridurre l'impatto negativo sulla salute dell’esposizione ad una classe di inquinanti ambientali chiamati Interferenti Endocrini. In particolare il progetto si focalizza sulla coppia madre-infante e sull’analisi del latte materno (e di quello artificiale) e della crescita e sviluppo del bambino durante il primo anno di vita. I primi risultati di questa analisi evidenziano la presenza di bisfenolo e di ftalati e questo ha portato ad elaborare una campagna di prevenzione in modo da aiutare le mamme a limitare l'esposizione a questi inquinanti ambientali.

Questa campagna di sensibilizzazione è stata condotta anche all'interno delle scuole secondarie di primo e secondo grado, per portare il messaggio del progetto Life MILCH anche ai più giovani.

L'Università di Cagliari ha avviato la sua campagna di sensibilizzazione nelle scuole grazie ad azioni mirate portate avanti dalla prof.ssa Angelica Dessì e dalle dott.sse Silvia Petza e Roberta Pintus.

Il progetto Europeo Life MILCH(LIFE 18 ENV/IT/000460) promuove la prevenzione e la riduzione dell'esposizione agli attraverso la costituzione di focus group/tavoli di lavoro finalizzati alla sottoscrizione di accordi di impegno volontario da parte delle imprese aderenti.
Tali accordi rappresentano un passo decisivo verso la costruzione di un'alleanza tra comunità scientifica e comparto produttivo impegnati nella mitigazione degli Interferenti Endocrini.
Il 26 Ottobre a Cagliari, durante il 21° International Workshop on Neonatology and Pediatrics, si è svolto un focus group, condotto dall'esperta facilitatrice dott.ssa Cinzia Zugolaro (SFERALAB) che si è aperto con una sintesi dei risultati più significativi emersi dalla ricerca condotta nell'ambito del progetto LIFE MILCH, per giungere alla sottoscrizione dell'accordo quale strumento volontario di tutela della salute pubblica e dell'ambiente.
Alcune aziende tra le presenti, tra cui Buona SPA Società Benefit (rappresentata in questa intervista dal dott. Pietro Armento, Medical Marketing Manager) ed Epinutracell (rappresentata dal dott. Giorgio Terziani in questo focus group) si sono impegnate a firmare un accordo scritto con il progetto LIFE MILCH, volto a testare gli Interferenti Endocrini ed a ridurre/abolire la loro presenza.

BETTER4U is a 4-year Horizon Europe funded project (2023-2027) that aims to develop comprehensive research and innovative interventions to address and reverse the widespread increase in and weight gain. The project seeks to better understand how , , and other factors may influence weight gain. Using this knowledge, the project will develop, evaluate and promote sustainable personalised lifestyle interventions to address and prevent obesity across the lifespan. The University of Cagliari focuses on the component of this study, exploring how the metabolomic profiles reflect and predict treatment responses.

Neonatal brain injury: when it begins and possible outcomes. The brain injury that occurs is something we don't see, but it's often already underway. Neuroprotective measures we can offer to prevent brain problems in newborns begin with maternal care and include the timely administration of medications or even hormones that have been shown to have beneficial effects (e.g., melatonin). However, careful monitoring of the newborn by experienced neonatologists and pediatric and neonatal neurologists is crucial because it can quickly reveal small signs, such as in the baby's movements, that can develop into serious problems.
