🔍FOCUS ON FRAILTY WEEK 1 – A is for Adjustment 🧑‍🦽‍➡️

ADAPT TO CHANGES...
Our bodies and minds inevitably change as we age. It’s important to adapt to these changes to stay healthy & happy.
Use aids if needed. Don’t worry about what other people think. Take what is on offer to help you maintain your independence. Consider a walking aid if you’ve had falls or issues with your balance. Get these aids checked regularly for wear and tear!
It’s not “giving in”, it’s staying safe to enjoy life.

MODIFY YOUR ENVIRONMENT...
Look at changes at home including removing rugs to avoid falls, makes sure there is adequate lighting and heating. Arranging the kitchen to get easy access to the things you use the most. Think about how safe it is getting in and out of the shower or bath and get adaptations like rails if needed.
Consider an Occupational Therapy assessment or speak to a local supplier for advice.

PACE YOURSELF...
Think of your body having a battery and only so much energy to do daily tasks, use this energy wisely to enjoy life.
It’s ok to have small naps to recharge, just don’t sleep in your chair all day!

THINKING AHEAD...
We all need to plan what we want from our healthcare, no matter how old we are.
Be prepared & in control of your health.
Now is the time to think about all those tasks you been putting off like writing a Will & Lasting Power of Attorney.
Consider what you want to happen and where you want to be looked after if you are unwell when you may not be able to express these wishes.

🔎FOCUS ON FRAILTY🔍
As part of our Focus On Frailty, we’re proud to introduce our Frailty Team — a group of healthcare professionals focused on proactive and preventative care to reduce the impact of frailty in our community.

Our mission is to help individuals stay well by enhancing overall wellbeing and reducing common stressors such as hospital admissions and falls. We offer patients a Comprehensive Geriatric Assessment, which helps us understand their unique needs and provide tailored support.

The team provides expert advice, makes referrals, signposts to relevant services, and works closely with all healthcare and support organisations across Jersey. We are also engaged in international benchmarking to ensure we deliver the best possible service and follow global best practices.

Ultimately, our work aligns with the Family Nursing & Home Care ethos — supporting people to “live well and age well.”

🔍FOCUS ON FRAILTY 18th Nov – 23rd Dec 2025🔍

Over the next six weeks we will be featuring the work of our Frailty team and offering advice on ways to stay ACTIVE as we get older, to avoid frailty and reduce the risk of falls.

Starting today, listen out for members of the team who will be on BBC Radio Jersey’s morning show every Tuesday over the next six weeks, offering advice to listeners on ways to stay ACTIVE!

All of us are more at risk of falling as we age; it’s a big cause of hospital admissions and can result in serious injuries and long-term complications. Falling can also contribute to a loss of confidence and independence.

The term ‘Frailty' describes someone’s overall ability to recover from illness or injury. Frail people don’t bounce back as quickly from illness and accidents and this can impact on their long-term health and wellbeing. In practice, being frail means a relatively minor health problem, such as a urinary tract infection, can have a severe long term impact on someone’s health and wellbeing.

The Frailty team have created the ACTIVE mnemonic to help people reduce their risk of falling and frailty. This helps THEM explain some of the simple actions patients can take and helps PATIENTS remember.

🤩COME AND JOIN OUR COMMUNITY NURSING TEAM 🤩
We are now recruiting for Grade 4 Community Staff Nurses (full or part time). Our Community Nurses provide expert care to patients over the age of 18 in individual homes, residences and clinic settings. They support islanders to help avoid the onset and reduce the impact of illness and try to limit disability, incapacity and shorten the duration of illnesses. We also support the management of long-term conditions and palliative and end of life care.
Our Community Nurses are supported by specialist nurses and senior health care assistants – they have the courage to do things differently and strive to provide innovation and new levels of care in the community.
We support our teams and offer regular clinical and safeguarding supervision, in house & external training opportunities for continued professional development. We have regular social events and continually develop our Wellbeing initiatives.
Previous community nursing experience is not essential. However, you will need to be a registered nurse with a full driving licence and your own transport.

With the help of our amazing Palliative Care Support Worker, Megan, the children had a fantastic time carving pumpkins and getting into the Halloween spirit. 👻🕷
