Guide wire retention is classed as a never event in the UK and an adverse event in the US both nationally reportable incidents and classed as the 3rd most commonly retained foreign object. Reported as a 1 in 10 RFO occurrence by the California department of public health. “A serious incident that is wholly preventable” with a 1:3000⁴ occurrence and a 35% mortality rate³ occurring only through human error. A retained guidewire is a serious reportable event (SRE) and included in the national reporting and litigation system (NRLS) within NHS improvement.
