ADM - Recession coverage (NCCL) VISTA approach to tunnelling for root recession coverage.

Before and after 2 weeks of healing between photos!

Here, I performed 13teeth and one implant soft tissue grafting.

The recession of gums can be progressive and cause sensitivity as well as loss of tooth root structure.

This minimally invasive approach allows the gums to heal in the correct position as well as changing biotope from thin to thick and preventing future relapse.

Surgery was performed under IV sedation and took 4 hours start to finish.

Extraction a d Immediate implant.

This patient had previously suffered with a failed root canal treatment and associated infection.

The were digitally planned and after atraumatic extraction we placed the implant with a 3D printed surgical guide.

You can see the precision of guided surgery the implant has been placed directly in the planned position. Thus is tricky as often the implant will want to slip into one of the root spaces.

See how we have used Zero bone loss concept of immediacy to preserve the soft tissues.

Not bad for a freehand placement, and implant appears to be healthy with stable bone levels on a chairside made temporary e.Max on tibase.... 10 years ago! (Although now I would use Zirconia to e.Max)

Following the protocol of minimally invasive overlay crowns to rebuild and restore heavily broken down LR5/6.

I first removed all the caries (decay) and then used an indicator to confirm the removal of all addicted dentine. IDS was performed, and subsequent same visit crowns adhesively bonded to the teeth to restore their function.

Ceramic on 🔥

The process of making dental crowns has been perfected since Ancient Egypt handmade crowns and bridges from gold wire and teeth.

Todays techniques have been perfected of many decades, so much so that we can now produce and 'fire' cermaic crowns chairside!.

This is not new technology, but it is still awe-inspiring ❤️ for example, this glowing E.max(LiSi) crown I processed today... looks amazing when it is cooling down from close to 1000°c

What is next for dentistry, 3D printing is progressing at lighting speed. I have been lucky to have been involved since it was a very new technology to.today where we can print implant crowns and permanent veneers!

It is an exciting time to be in dentistry!

Tongue Tie Tuesday

A "tongue tie" or lingual frenulum is a normal part of the anatomy of the tongue. However, in some instances, this tie can be more "tethered" than It should.

Restricted tongue movement
Affecting speech and function

This can be treated if diagnosed at birth as babies can have trouble latching on... However, a lot do get missed and can be treated with a dental laser
Waterlase, which results in clean cuts and no bleeding.

I always prefer using waterlaser laser
The result is much cleaner, less inflammation, and much quicker healing.
Often, patients have minimal pain and swelling, if any.

"Gum Recession"

There are many causes of recession... I am going to focus on Miller Class 1 treatment of mild - moderate recession.

This lady has a particularly thin gingival phenotype, and due to many years of manual tooth brushing caused some noticable recession.

Traditionally treated with topical flouride and high concentration flouride toothpastes. More extreme options are direct composites.

We had tried topical therapy, but the sensitivity was significant. Therefore, CTG harvested from the palate was used in the tunnelling technique to coronally advance the tissues as taught to me by Ricardo Kern & Otto Zhur.

EDTA for 2 minutes was used to condition the root surface. The de-epithelised CTG was secured with non resorbable sutures on either side.. and the flap secured with double cross over sutures as described by Otto Zhur.
