Treating the whole patient

I try to be thorough and address all the signs and symptoms even those outside of Gynecology and Obstetrics. It's not uncommon that ladies have other issues such as stomach pain from an ulcer , headaches, back pains, depression. I will manage some of these additional issues and refer to the relevant specialist where indicated as I try to help the whole patient. As eg you can't just treat the fibroids but ignore the depression that the fibroid is causing.

Even though we recommend starting pap smears at age 21. As some females may have been exposed to the HPV at an earlier age, Papsmears may need to be started earlier in order to prevent ladies developing Cervical Cancer in there 20s and 30s etc.

Uterine fibroids

These non cancerous growths in the womb cause no major symptoms in most women.
However they can cause heavy periods, pelvic pain before the period that tends to improve during the first few days of the period, back pain, urinary frequency.
They may cause infertility based on their location. In pregnancy there may be no ill effects from the fibroids, for most ladies fibroids in pregnancy causes no problems or there may be an increased risk of miscarriage, pain from degeneration ( fibroid breaking down) or premature labour.
Most ladies who have fibroids do NOT need surgery.

Pcos is diagnosed
based on 2 out of 3 criteria:
1) Irregular periods or lack of regular ovulation, absent ovulation.
2) Signs of excess male hormone( testosterone/androgens) such as acne or oily skin or balding in central region of the hair , seborrheic dermatitis, facial hair.
3) Polycystic ovaries on ultrasound may be present. Absence of Polycystic ovaries does NOT mean that Pcos is absent. When we make the diagnosis, we look at the symptoms, do hormonal blood tests and a pelvic ultrasound.
