Abstract: A 68-year-old male with a history of untreated HCV presented with a left lower extremity ulcer that had progressively worsened over 4 days after minor trauma. Examination revealed a 2x3 cm purulent ulcer with erythematous rim. HCV viral load and genotype analysis revealed genotype 1A with PCR showing viral counts of 9,506,048 and cryoglobulinemia. With worsening ulcer and failure of IV antibiotic therapy, skin biopsy showed acanthotic epidermis with superficial and deep perivascular lymphoplasmacytic dermatitis admixed with mild neutrophilic infiltrate. Patient was subsequently started on ledipasvir/sofosbuvir and prednisone with high suspicion of pyoderma gangrenosum. The case highlights the importance of considering pyoderma gangrenosum in patients with HCV and non-healing ulcers that fail to respond to antibiotic therapy, and demonstrates successful treatment with direct-acting antivirals and corticosteroids.

Abstract: A 78-year-old farmer with a history of diabetes mellitus and rheumatoid arthritis, controlled with metformin and prednisone 20 mg daily, presented with erythematous nodular skin lesions over the back of his right thigh of 1-week duration. He had not had any opportunistic infection previously. Two lesions were cystic and aspirated. Fluid culture grew Mycobacterium chelonae. This case illustrates the importance of considering atypical mycobacterial infections in immunocompromised patients with nodular skin lesions and the need for appropriate diagnostic workup including culture and susceptibility testing.

Abstract: Minocycline, a tetracycline antibiotic commonly used for acne and various infections, can cause cutaneous hyperpigmentation as an adverse effect. We present a case of minocycline-induced skin discoloration and review the literature on pathogenesis, clinical presentation, and management of this condition. Early recognition is important to prevent unnecessary diagnostic procedures and to guide appropriate management.

Abstract: Prosthetic joint infections are typically caused by staphylococci and streptococci. We report a rare case of prosthetic joint infection due to Listeria monocytogenes in an immunocompromised host. This case highlights the importance of considering unusual pathogens in prosthetic joint infections, particularly in patients with risk factors, and the need for prolonged combination antibiotic therapy and surgical intervention.
