Today is the International Day of Women & Girls in Science! To celebrate, we are honoring the life and legacy of Sister M. Yvonne Jenn. She was one of the nine incorporators of the American Registry of Inhalation Therapists (now NBRC). In November 1960, Sister M. Yvonne Jenn became the first Registered Inhalation Therapist in the USA.

She was a CRNA and served as a program director of the Franciscan Skemp School of Nurse Anesthesia in La Crosse, WI. Sister Yvonne served on the AAIT Board of Directors and served as Executive Director of the American Registry of Inhalation Therapists. The NBRC’s highest award is named in her honor: the Sister Mary Yvonne Jenn, CRNA RRT Lifetime Achievement Award.

①医療ガスの相対湿度はほぼ0%。深部温37度の生体の粘膜を通過する時に、ガスは、確実に粘膜細胞の水分を奪ってゆく。それは粘膜障害の原因となる。②一方、気管チューブ内は、そこを通過するガスを加湿する機能はほぼない（加温はされるかもしれないが）。喀痰があれば、その水分を奪い、喀痰の粘性が増す。ひいてはチューブ閉塞の原因となる。③多くの場合、室温＜体温である。体外から、体内にガスが移動する場合、多くの場合は加温されて相対湿度は低下する。このため粘膜障害や喀痰の粘性が増す。④体内に入ってゆく直前の口元で、少なくとも結露（＝相対湿度100%）を確認せよと言われるゆえんである。ただし体に入って、すぐに相対湿度100%を切らせないためにも、呼吸回路を加温して、絶対湿度も保たれなければならない。

“Rapid”とは、そういう事である。”Express”ではないのである。逆に”Express”な対応が必要にならないよう、しなければならない。ところで、ここには”Emergency”は出てこない。”Emergency”は、スピード、時間的切迫性というより、事件、事故、生命を脅かすような災害的な、混乱、重病のニュアンスらしい。でも、急いで対応する必要はあるだろうけど。

Premature decisions about irreversibility of lung injury should be avoided.
