ARTHRITIS

"Arthritis" literally means joint inflammation. Joints are places where two bones meet, such as your elbow or knee. There are many different types of arthritis with different causes and treatments. In some types, other organs, such as your eyes, heart, or skin, can also be affected.

Different types of arthritis exist, each with different causes including wear and tear, infections and underlying diseases.
Symptoms include pain, swelling, reduced range of motion and stiffness.
Medication, physiotherapy or sometimes surgery helps reduce symptoms and improve quality of life.

SYMPTOMS
-Pain areas: in the joints, ankle, back, fingers, hands, muscles, neck, or wrist
-Pain types: can be intermittent or sharp
-Pain circumstances: can occur while sitting
-Muscular: decreased range of motion, difficulty walking, or muscle weakness
-Joints: stiffness, tenderness, or swelling

URINARY TRACT INFECTION(UTI)
A urinary tract infection (UTI) is an infection in any part of the urinary system. The urinary system includes the kidneys, ureters, bladder and urethra. Most infections involve the lower urinary tract — the bladder and the urethra.

Women are at greater risk of developing a UTI than are men. If an infection is limited to the bladder, it can be painful and annoying. But serious health problems can result if a UTI spreads to the kidneys.

Health care providers often treat urinary tract infections with antibiotics. You can also take steps to lower the chance of getting a UTI in the first place.
Symptoms

UTIs don't always cause symptoms. When they do, they may include:

-A strong urge to urinate that doesn't go away
-A burning feeling when urinating
-Urinating often, and passing small amounts of urine
-Urine that looks cloudy
-Urine that appears red, bright pink or cola-colored — signs of blood in the urine
-Strong-smelling urine
-Pelvic pain, in women — especially in the center of the pelvis and around the area of the p***c bone.

In older adults, UTIs may be overlooked or mistaken for other conditions.

RISK FACTORS
UTIs are common in women. Many women experience more than one UTI during their lifetimes.

Risk factors for UTIs that are specific to women include:

-Female anatomy. Women have a shorter urethra than men do. As a result, there's less distance for bacteria to travel to reach the bladder.
-Sexual activity. Being s*xually active tends to lead to more UTIs.
-Having a new s*xual partner also increases risk.
-Certain types of birth control. Using diaphragms for birth control may increase the risk of UTIs. Using s***micidal agents also can increase risk.
-Menopause. After menopause, a decline in circulating estrogen causes changes in the urinary tract. The changes can increase the risk of UTIs.

Other risk factors for UTIs include:

-Urinary tract problems. Babies born with problems with their urinary tracts may have trouble urinating. Urine can back up in the urethra, which can cause UTIs.
-Blockages in the urinary tract. Kidney stones or an enlarged prostate can trap urine in the bladder. As a result, risk of UTIs is higher.
-A suppressed immune system. Diabetes and other diseases can impair the immune system — the body's defense against germs. This can increase the risk of UTIs.
-Catheter use. People who can't urinate on their own often must use a tube, called a catheter, to urinate. Using a catheter increases the risk of UTIs. Catheters may be used by people who are in the hospital. They may also be used by people who have neurological problems that make it difficult to control urination or who are paralyzed.
-A recent urinary procedure. Urinary surgery or an exam of your urinary tract that involves medical instruments can both increase the risk of developing a UTI.

COMPLICATIONS
When treated promptly and properly, lower urinary tract infections rarely lead to complications. But left untreated, UTIs can cause serious health problems.

COMPLICATIONS OFA UTI MAY INCLUDE:

-Repeated infections, which means you have two or more UTIs within six months or three or more within a year. Women are especially prone to having repeated infections.
-Permanent kidney damage from a kidney infection due to an untreated UTI.
-Delivering a low birth weight or premature infant when a UTI occurs during pregnancy.
-A narrowed urethra in men from having repeated infections of the urethra.
-Sepsis, a potentially life-threatening complication of an infection. This is a risk especially if the infection travels up the urinary tract to the kidneys.

PREVENTION
These steps may help lower the risk of UTIs:

-Drink plenty of liquids, especially water. Drinking water helps dilute the urine. That leads to urinating more often — allowing bacteria to be flushed from the urinary tract before an infection can begin.
-Try cranberry juice. Studies that look into whether cranberry juice prevents UTIs aren't final. However, drinking cranberry juice is likely not harmful.
-Wipe from front to back. Do this after urinating and after a bowel movement. It helps prevent the spread of bacteria from the a**s to the va**na and urethra.
-Empty your bladder soon after having s*x. Also drink a full glass of water to help flush bacteria.
-Avoid potentially irritating feminine products. Using them in the ge***al area can irritate the urethra. These products include deodorant sprays, douches and powders.
-Change your birth control method. Diaphragms, unlubricated condoms or condoms treated with s***micide can contribute to bacterial growth.

Male infertility

Male infertility can be caused by low s***m production, abnormal s***m function or blockages that prevent the delivery of s***m. Illnesses, injuries, chronic health problems, lifestyle choices and other factors may contribute to male infertility.

The inability to conceive a child can be stressful and frustrating, but a number of treatments are available for male infertility.

SYMTOMS

The main sign of male infertility is the inability to conceive a child. There may be no other obvious signs or symptoms.

In some cases, however, an underlying problem such as an inherited disorder, hormonal imbalance, dilated veins around the testicle or a condition that blocks the passage of s***m causes signs and symptoms. Signs and symptoms you may notice include:

-Problems with s*xual function — for example, difficulty with ej*******on or small volumes of fluid ej******ed, reduced s*xual desire, or difficulty maintaining an er****on (erectile dysfunction)
-Pain, swelling or a lump in the testicle area
-Recurrent respiratory infections
-Inability to smell
-Abnormal breast growth (gynecomastia)
-Decreased facial or body hair or other signs of a chromosomal or hormonal abnormality
-A lower than normal s***m count (fewer than 15 million s***m per milliliter of semen or a total s***m count of less than 39 million per ej*****te).

WHEN TO SEE A DOCTOR
See a doctor if you have been unable to conceive a child after a year of regular, unprotected in*******se or sooner if you have any of the following:

-Er****on or ej*******on problems, low s*x drive, or other problems with s*xual function
-Pain, discomfort, a lump or swelling in the testicle area
-A history of testicle, prostate or s*xual problems
-A groin, testicle, p***s or sc***um surgery
-A partner over age 35

RISK FACTORS
Risk factors linked to male infertility include:

-Smoking to***co
-Using alcohol
-Using certain illicit drugs
-Being overweight
-Having certain past or present infections
-Being exposed to toxins
-Overheating the testicles
-Having experienced trauma to the testicles
-Having a prior vasectomy or major abdominal or pelvic surgery
-Having a history of undescended testicles
-Being born with a fertility disorder or having a blood relative with a fertility disorder
-Having certain medical conditions, including tumors and chronic illnesses, such as sickle cell disease
-Taking certain medications or undergoing medical treatments, such as surgery or radiation used for treating cancer

COMPLICATIONS
Complications of male infertility can include:

-Stress and relationship difficulties related to the inability to have a child
-Expensive and involved reproductive techniques
-Increased risk of testicular cancer, melanoma, colon cancer and prostate cancer.

PREVENTION.
Male infertility isn't always preventable. However, you can try to avoid some known causes of male infertility. For example:

-Don't smoke.
-Limit or abstain from alcohol.
-Steer clear of illicit drugs.
-Maintain a healthy weight.
-Don't get a vasectomy.
-Avoid things that lead to prolonged heat for the testicles.
-Reduce stress.
-Avoid exposure to pesticides, heavy metals and other toxins.

HIGH BLOOD PRESSURE
(HYPERTENSION)
OVERVIEW

Hypertension (high blood pressure) is when the pressure in your blood vessels is too high (140/90 mmHg or higher).
Based on recommendation of the JNC 7 ,the classification for Blood pressure for adults aged 18 years or older bas been as follow:

_Normal: systolic lower than 120 mmHg, diastolic lower than 80 mmHg.
_pre hypertension; Systolic 120-139 mmHg, diastolic 80-89 mmHg.
_Stage 1: systolic 140- 159 mmHg, diastolic 90-99mmHg.
_stage 2: systolic 160 mmHg or greater ,diastolic 100 mmHg or greater.
High blood pressure is common but can be serious if not treated.
People with high blood pressure may not feel symptoms.
The only way to know is to get your blood pressure checked.

Hypertension may be primary, which may develop as a result of environmental or genetic causes, or secondary, which has multiple etiologies, including renal, vascular, and endocrine causes.
Primary or essential hypertension accounts for 90-95% of adult cases, and secondary hypertension accounts for 2-10% of cases.

DIAGNOSIS OF HYPERTENSION

The evaluation of hypertension involves accurately measuring the patient’s blood pressure, performing a focused medical history and physical examination, and obtaining results of routine laboratory studies.

2)PHARMACOLOGICAL MANAGEMENT

JNC 7 recommendations to lower BP and decrease cardiovascular disease risk include the following, with greater results achieved when 2 or more lifestyle modifications are combined;

-Weight loss (range of approximate systolic BP reduction 20 mm Hg per 10 kg)

-Limit alcohol intake to no more than 1 oz (30 mL) of ethanol per day for men or 0.5 oz (15 mL) of ethanol per day for women and people of lighter weight (range of approximate SBP reduction, 2-4 mm Hg)

-Reduce sodium intake to no more than 100 mmol/day (2.4 g sodium or 6 g sodium chloride; range of approximate SBP reduction, 2-8 mm Hg)[8]

-Maintain adequate intake of dietary potassium (approximately 90 mmol/day)

-Maintain adequate intake of dietary calcium and magnesium for general health

-Stop smoking and reduce intake of dietary saturated fat and cholesterol for overall cardiovascular health

-Engage in aerobic exercise at least 30 minutes daily for most days (range of approximate SBP reduction, 4-9 mm Hg).
