* NEJM 신규 RCT: Ketamine vs Etomidate - RSI 유도제 비교

중환자 기관삽관 시 ketamine과 etomidate를 비교한 대규모 다기관 RCT(n=2,007)가 NEJM에 발표되었습니다.

주요 결과: 7일 사망률에서 두 약제 간 유의한 차이 없음(ketamine 21.2% vs etomidate 18.4%, p=0.12). 그러나 ketamine군에서 심혈관 허탈(cardiovascular collapse) 발생이 유의하게 높았습니다(3.7% vs 1.6%, p=0.01).

** 외상의료진 주목: 이 연구는 외상환자를 명시적으로 제외했습니다. 출혈성 쇼크와 패혈성 쇼크의 병태생리가 다르고, 기존 외상 특이적 연구들(Upchurch 2017, Korean study 2023)에서는 두 약제 간 사망률 차이가 없었습니다. EAST 가이드라인도 특정 약제를 권고하지 않습니다. 따라서 외상환자에게는 본 연구 결과를 직접 적용하기보다, 환자 상태(TBI, 출혈성 쇼크, 다발성 외상)에 따른 개별화된 선택이 적절합니다.

Critical Care에 실린 흥미로운 논문 공유합니다. London's Air Ambulance가 30년 이상 축적한 prehospital RT 데이터 600건 이상을 분석해서 실용적인 decision framework를 제시했네요. 가장 인상적인 부분은 손상 위치와 ECG rhythm이라는 간단한 bedside information만으로도 meaningful한 예측이 가능하다는 점입니다. Cardiac box 칼상처는 isolated tamponade가 거의 50%인 반면, 총상은 25% 정도로 낮고 나머지는 대부분 exsanguination이었습니다. ECG rhythm의 prognostic value도 명확했는데, organized rhythm에서 tamponade 생존율이 60%인 반면 asystole에서는 10%에 불과했고, exsanguination + asystole은 사실상 futile했습니다. 결국 cardiac box 관통상 + organized rhythm이면 immediate RT, 그 외에는 transfusion과 hemorrhage control 먼저라는 straightforward한 알고리즘이 나오네요. Prehospital setting에서 적용 가능한 practical guidance라고 생각됩니다.

Many EMS protocols recommend lidocaine before intraosseous (IO) infusion to reduce pain — but how well does it actually work?

Review of published studies shows:
• IO flushing is uniformly painful
• Humeral IOs hurt less than tibial
• Flushing causes more pain than insertion
• Lidocaine helps some, but data are limited

In the most detailed trials, 40 mg IO lidocaine over 2 min, followed by a rapid saline flush and 20 mg more, gave the best pain control for humeral sites.

Bottom line: For alert patients, lidocaine may help and won’t hurt—but the evidence remains thin.
