👣 About orthotics therapy

# If you are having difficulty moving normally due to an injury or an illness, your physical therapist may fit you with an orthotic to help align your feet and body properly. So what exactly is an orthotic? An orthotic is an orthopedic appliance or apparatus used to support, align, prevent, or correct deformities or to improve the function of movable parts of the body. Simply put, an orthotic is any device that is applied to your body that is meant to help you move and function better. There are several different types of orthotics that your physical therapist may use.

👣 AFO - Ankle Foot Orthosis; orthotic device for the lower limb that encloses the ankle and foot and does not extend above the knee. This is commonly used in foot drop where weakness of the anterior tibialis prevents you from lifting your foot and toes up off the floor.

👣 Cervical brace - a rigid plastic orthosis that encircles the neck and supports the chin and the back of the head.

👣 Dynamic splint - a dynamic splint is a device that surrounds a joint to support it but moves to align your joint while stretching a specific part of it. For example, if you are experiencing elbow tightness after an arm fracture, your PT may prescribe a dynamic splint to help strethc your arm out while supporting it. The splint provides a low-load, long-duration stretch to your body part.

👣 KAFO - Knee Ankle Foot Orthosis; orthotic device for the lower limb that extends from above the knee to the ankle and foot.

👣 LSO - Lumbosacral Orthosis; spinal orthosis that encircles the body in the lumbosacral region. This is often used after spinal surgery or to treat low back pain from spinal stenosis

# Orthotics may be recommended by your podiatrist specialist to help align your body or to help support a joint or joints. This can improve your overall functional mobility by helping to support your body after injury or illness that results in loss of muscular control or joint mobility.

👣 SCOLIOSIS

Scoliosis is a sideways curvature of the spine that occurs most often during the growth spurt just before puberty. While scoliosis can be caused by conditions such as cerebral palsy and muscular dystrophy, the cause of most scoliosis is unknown. About 3% of adolescents have scoliosis.

Most cases of scoliosis are mild, but some spine deformities continue to get more severe as children grow. Severe scoliosis can be disabling. An especially severe spinal curve can reduce the amount of space within the chest, making it difficult for the lungs to function properly.

Children who have mild scoliosis are monitored closely, usually with X-rays, to see if the curve is getting worse. In many cases, no treatment is necessary. Some children will need to wear a brace to stop the curve from worsening. Others may need surgery to keep the scoliosis from worsening and to straighten severe cases of scoliosis.

👣 Symptoms

Signs and symptoms of scoliosis may include:

* Uneven shoulders
* One shoulder blade that appears more prominent than the other
* Uneven waist
* One hip higher than the other

If a scoliosis curve gets worse, the spine will also rotate or twist, in addition to curving side to side. This causes the ribs on one side of the body to stick out farther than on the other side.

👣 When to see a doctor

Go to your doctor if you notice signs or symptoms of scoliosis in your child. Mild curves, however, can develop without the parent or child knowing it because they appear gradually and usually don't cause pain. Occasionally, teachers, friends and sports teammates are the first to notice a child's scoliosis.

👣 Diabetes Foot Care Guidelines

* Diabetes can be dangerous to your feet—even a small cut can produce serious consequences. Diabetes may cause nerve damage that takes away the feeling in your feet. Diabetes may also reduce blood flow to the feet, making it harder to heal an injury or resist infection. Because of these problems, you may not notice a foreign object in your shoe. As a result, you could develop a blister or a sore. This could lead to an infection or a nonhealing wound that could put you at risk for an amputation.

To avoid serious foot problems that could result in losing a toe, foot or leg, follow these guidelines.

* Inspect your feet daily. Check for cuts, blisters, redness, swelling or nail problems. Use a magnifying hand mirror to look at the bottom of your feet. Call your doctor if you notice anything.

* Bathe feet in lukewarm, never hot, water. Keep your feet clean by washing them daily. Use only lukewarm water—the temperature you would use on a newborn baby.

* Be gentle when bathing your feet. Wash them using a soft washcloth or sponge. Dry by blotting or patting and carefully dry between the toes.

* Moisturize your feet but not between your toes. Use a moisturizer daily to keep dry skin from itching or cracking. But don't moisturize between the toes—that could encourage a fungal infection.

* Cut nails carefully. Cut them straight across and file the edges. Don’t cut nails too short, as this could lead to ingrown toenails. If you have concerns about your nails, consult your doctor.

* Never treat corns or calluses yourself. No “bathroom surgery” or medicated pads. Visit your doctor for appropriate treatment.

* Wear clean, dry socks. Change them daily.

' * Consider socks made specifically for patients living with diabetes. These socks have extra cushioning, do not have elastic tops, are higher than the ankle and are made from fibers that wick moisture away from the skin.

* Wear socks to bed. If your feet get cold at night, wear socks. Never use a heating pad or a hot water bottle.

* Shake out your shoes and feel the inside before wearing. Remember, your feet may not be able to feel a pebble or other foreign object, so always inspect your shoes before putting them on.

* Keep your feet warm and dry. Don’t let your feet get wet in snow or rain. Wear warm socks and shoes in winter.

* Consider using an antiperspirant on the soles of your feet. This is helpful if you have excessive sweating of the feet.

* Never walk barefoot. Not even at home! Always wear shoes or slippers. You could step on something and get a scratch or cut.

* Take care of your diabetes. Keep your blood sugar levels under control.

* Do not smoke. Smoking restricts blood flow in your feet.

* Get periodic foot exams. Seeing your foot and ankle surgeon on a regular basis can help prevent the foot complications of diabetes.

* If a child has a discrepancy in leg length, it will usually become obvious to parents as they watch the child grow and begin to crawl and walk. Some children are born with legs of different lengths. In other cases, illness or injury causes a discrepancy in length to develop over time. While a slight difference in leg length may not cause symptoms, a significant difference can cause a noticeable limp and make it difficult for a child to run and play.

* Treatment for a discrepancy depends upon the severity. In many cases, a minor difference in leg length can be evened out by wearing a lift in one shoe. A child with a more significant difference, however, may benefit from surgery to make his or her legs the same length. This can be done a number of ways, but is most often accomplished through a procedure that slows or stops growth in the longer leg.

* What's causing my symptoms?

* Is my condition likely temporary or chronic?
* What treatment do you recommend?
* Do I need to restrict my activities?
* Are there brochures or other printed material I can have? What websites do you recommend?
* What to expect from your doctor

👣 Your doctor is likely to ask you a number of questions, including:

* What type of shoes do you wear?
* What activities do you do?
* Does your daily routine involve a lot of walking or standing?
* Do you often go barefoot? On what types of surfaces?
* Are your symptoms continuous or occasional?
* How severe are your symptoms?
* What, if anything, seems to improve your symptoms?
* What, if anything, appears to worsen your symptoms?
* What you can do in the meantime
* While you're waiting to see your doctor, rest your foot as much as possible and wear properly fitting shoes. Over-the-counter pain relievers might help ease your discomfort.

👣 How can diabetes affect my feet?

* Over time, diabetes may cause nerve damage, also called diabetic neuropathy, that can cause tingling and pain, and can make you lose feeling in your feet. When you lose feeling in your feet, you may not feel a pebble inside your sock or a blister on your foot, which can lead to cuts and sores. Cuts and sores can become infected.

* Diabetes also can lower the amount of blood flow in your feet. Not having enough blood flowing to your legs and feet can make it hard for a sore or an infection to heal. Sometimes, a bad infection never heals. The infection might lead to gangrene.

* Gangrene and foot ulcers that do not get better with treatment can lead to an amputation of your toe, foot, or part of your leg. A surgeon may perform an amputation to prevent a bad infection from spreading to the rest of your body, and to save your life. Good foot care is very important to prevent serious infections and gangrene.

* Although rare, nerve damage from diabetes can lead to changes in the shape of your feet, such as Charcot’s foot. Charcot’s foot may start with redness, warmth, and swelling. Later, bones in your feet and toes can shift or break, which can cause your feet to have an odd shape, such as a “rocker bottom.”

*Look for problems:
- such as cuts, sores,
- red spots swelling or fluid-filled blisters
- ingrown toenails, in which the edge of your nail grows into your skin
- corns or calluses, which are spots of rough skin caused by too much rubbing or pressure on the same spot
- plantar warts, which are flesh-colored growths on the bottom of the feet
- athlete’s foot
- warm spots

* If you have certain foot problems that make it more likely you will develop a sore on your foot, your doctor may recommend taking the temperature of the skin on different parts of your feet. A “hot spot” can be the first sign that a blister or an ulcer is starting.
