This case was referred to me by one of my colleagues after she experienced an incident involving a separated instrument.

In cases of separated instruments, there are several methods for retrieval; at the same time, we should consider preserving as much of the tooth structure as possible.

In this case, the instrument was removed by using a large-gauge injection needle that fit the separated instrument and a file to engage and withdraw it.

RCT was completed in the same visit.

A case of failed calcium hydroxide direct pulp cap, the patient presented with pulp necrosis and asymptomatic apical periodontitis.

CBCT revealed a c-shape canal with 2 portal of exits, buccal and palatal.

The buccal was an open apex, it was obturated with BC putty and the palatal with WVC and BC sealer.

NS-RCT was done over 2 visits with calcium hydroxide as an intracanal medicament.
