Upper Blepharoplasty – Volumetric Contouring Concept 2.0 with Posterior Neck Donor Area

Dr. Nabil Fakih is the inventor of the volumetric contouring concept of the entire orbital rim. He has consistently opposed the routine resection of periocular fat, as excessive fat removal leads, over time, to orbital skeletonization as the patient ages.

He has always advocated for a healthy, natural orbital rim contour, which is achieved by preserving the appropriate amount of fat—a key element in creating aesthetically pleasing eyes. Both excess and deficiency of fat are problematic: excessive fat results in a heavy, tired appearance, while insufficient fat produces a hollowed, skeletonized look.

With a forward-looking, long-term surgical philosophy, acknowledging that patients will continue to age, proper fat preservation and strategic redistribution to recreate harmonious orbital contours offers a far more durable and aesthetic outcome.

This patient represents a complex revision case, having undergone two previous surgeries with another surgeons. The challenge was to completely redesign the orbital contour. Fat was harvested from the posterior neck and strategically redistributed at the retroseptal level, as well as placed along the orbital bony rim using a taco graft technique. Additional work included intervention at the lateral eyebrow region to suspend the orbicularis retaining ligament (ORL), along with several other strategic refinements, including minimal skin excision and correction of a ptotic lacrimal gland and others.

We sincerely thank the patient for her trust and for allowing us to share her before-and-after journey. We are pleased to have been able to improve her outcome through a comprehensive, long-term, volumetric approach.

For their trust, and for giving me the passion to create and recreate joy on their faces. They know I do not possess divine powers, but they also know that I will always give my very best, guided by science, judgment, and compassion, as if their face were my own.

In 2025 more than 25 published articles, 90 flights covering over 200,000 km, and 60 conferences, the mission remains the same: learning, teaching, and forever being a student.

With proper surgical technique and an appropriate healing protocol, the scars can become barely visible; however, the final outcome also depends on the patient’s skin type and individual healing response.

While incision results can never be guaranteed, I care deeply about scar quality and consistently apply meticulous technique and follow-up to ensure the incision lines are as discreet as possible.

For years, my philosophy has been rooted in the belief that true success is never individual, it is collective, and everyone who worked with me, knows very well this concept.

In a true workplace community, every team member plays an essential role in the success of others. Each person’s growth strengthens the whole, and no achievement stands alone. When one member moves forward, the entire community advances. When one struggles, the community does not turn away, it steps closer.

This philosophy is simple yet powerful: when someone pushes for you, you push for them. Support is not transactional; it is intentional. It is the understanding that lifting others does not diminish your own progress, it amplifies it.
