Dr. Nabil Fakih has been featured in Diario Medico, one of Spain’s leading medical journals, highlighting his international career, scientific leadership, and humanitarian commitment both inside and outside the operating room.

A Cranio-Maxillo-Facial surgeon and global reference in facial plastic surgery, Dr. Fakih specializes in facial surgery with keen interest in facelifts and eyelid surgery. During the war in Lebanon, he performed altruistic reconstructive surgeries for patients who had lost an eye or parts of their face.

Since December, he has been serving as President of the Spanish Society of Facial Plastic Surgery (SECPF) , where he promotes evidence-based medicine, multidisciplinary collaboration, and stricter regulation against medical malpractice and unqualified practice.

Eyelid surgery is fundamentally a procedure of design, balance, and artistic vision. The upper eyelid cannot be evaluated in isolation, as its aesthetics and function depend closely on eyebrow position, projection, and support. In facial surgery, every structure interacts with its surroundings, so each intervention must preserve overall facial harmony.
In this case, an upper blepharoplasty was initially planned. However, after detailed analysis, we first optimized eyebrow position with a brow lift to suspend the descent of the ROOF (Retro-Orbicularis Oculi Fat). This restored the architecture of the upper third of the face. After approximately three months—allowing proper tissue adaptation and reassessment—we performed the upper blepharoplasty with greater precision.
Dr. Nabil Fakih recommends staging both procedures whenever possible to maximize diagnostic and surgical accuracy. Although some patients require a single combined surgery, separating the procedures generally allows more refined control and a more personalized final result. Operating on medical colleagues and friends carries an additional and profound responsibility. It is not solely about technical skill; it requires a particular gift-the ability to unite science, sensitivity, and trust. When the person placing themselves in your hands deeply understands anatomy, surgical principles, and potential risks, the trust they extend acquires even greater meaning. 2025 Guide for Managing Vascular Occlusion with Exclusive Cutaneous Involvement

Prompt recognition and management of vascular occlusion secondary to hyaluronic acid fillers are essential to prevent irreversible tissue damage. This study reviews the key determinants of clinical outcomes, excluding ocular complications, and evaluates contemporary management protocols and advanced therapeutic strategies, proposing an updated practical guide for 2025.

All the evidence synthesized in this guide served as the foundation for the development of , a dedicated clinical support application designed to assist physicians in the prompt recognition and management of filler-related vascular complications.

The application translates current published evidence and expert-based algorithms into a structured, step-by-step decision-making tool. It provides real-time guidance on diagnosis, severity assessment, hyaluronidase dosing strategies, injection mapping, adjunctive therapies, follow-up protocols, and documentation.

By integrating evidence-based recommendations into an accessible digital platform, aims to reduce treatment delays, standardize emergency responses, minimize tissue loss, and ultimately improve patient safety and clinical outcomes.

📕 Low Dose or High Dose of Botulinum Toxin: What Is Its Effect?

A recent investigation has been published, and we are honored to be part of shaping the future of aesthetic medicine through robust, evidence-based research. Thanks to all co-authors for all the help and support.

Botulinum toxin demonstrates dual vascular mechanisms that are highly dependent on dosage. At higher doses, it increases tissue perfusion and promotes angiogenesis through the activation of vascular endothelial growth factor and nitric oxide pathways. These effects have shown therapeutic potential in ischemic flaps and in conditions such as Raynaud’s phenomenon. In contrast, at lower doses, botulinum toxin inhibits excessive cholinergic-mediated vasodilation and reduces neurogenic inflammation. This mechanism provides clinical benefits in disorders characterized by vascular hyperreactivity, including rosacea and hyperhidrosis. Although these effects are promising, they are tissue-specific and strictly dose-dependent, and results may vary according to the experimental model. The authors proposed that a concentration greater than 20 UI/mm should be considered a high dose, whereas lower concentrations were classified as low-dose botulinum toxin.

A lo largo de la vida pasan muchas personas. Con algunas compartes tu manera de trabajar, la ciencia, la pasión, el crecimiento. Y, con el tiempo, te reconocen, te escriben, te lo recuerdan. Lo hacen desde el corazón. Sin duda, es una sensación profundamente bonita sentirse reconocido de verdad, de corazón. Esa gratitud sincera te devuelve energía y te impulsa a seguir dando aún más a los demás.
