Severe TMJ Degenerative Osteoarthritis Case
Today I’m sharing a challenging temporomandibular joint (TMJ) case presenting with advanced degenerative osteoarthritic changes of the condyle.
This patient arrived with significant jaw pain, limited function, and chronic discomfort. Imaging revealed:
🔹 Bone marrow edema within the condylar head
🔹 Erosive changes and subchondral sclerosis
🔹 Flattening of the condylar head
🔹 A fluid-filled glenoid fossa indicating active inflammation
🔹 Complete disc displacement, with the disc torn, dilacerated, and non-functional
The flattening of the condyle has completely eliminated the functional joint space, leaving no room for a proper disc–joint relationship. This results in abnormal joint mechanics, persistent pain, and progressive degeneration if left untreated.
💡 These cases highlight the importance of:
▪ Early diagnosis
▪ Advanced imaging (CBCT/MRI)
▪ Conservative management to control inflammation
▪ Protecting the joint from further loading and deterioration
TMJ osteoarthritis is complex, but with a tailored treatment plan, many patients experience meaningful relief and improved function.
