That Integrative medical practitioners participate significantly on horizontal health-care equity in primary health-care dispensation. There are annual medical camps held through-out different districts of Lesotho where Integrative medical Practitioners are tasked in the organization of the medical camps, followed by (OPD-Consultations, Investigations, Diagnosis, Mx/Tx) during the day of the camp. Integrative medical practitioners also contribute educational lessons on Lesotho’s national broadcasting stations to strengthen and highlight Lesotho’s healthcare policy on the social determinants of health and on non-communicable diseases.

Integrative Medicine in Lesotho... An emerging patient-centric approach in Primary Health Care in Lesotho

INTEGRATIVE MEDICINE AS HEALTHCARE DEFENSE IN DEPTH.

Integrative medicine as defense in depth is a dynamic healthcare strategy that uses multiple professions as layers of overlapping defensive measures to protect and provide resistance against threats to health and wellbeing.

The aims of this approach are comparable to radical medicine based on critical healthcare principles that:
1. Healthcare focus is directed on the root cause of disease;
2. Healthcare is community-based and of collaborative care;
3. Healthcare is accepted as an autonomous social right for all;
4. And most significantly, the government exerts political will on healthcare reforms such as on the doctor-patient ratio equity and support on public-private-partnership.

The strategy of integrative medicine as defense in depth is based on principles of attrition, of deny and or delay of pathological processes. This move by secondary default is intended to buy time and create opportunities for universal healthcare response counter measures and or the re-deployment of health resources. The same principles are observed in the microcosm of health emergency response operations (HERO) with the following aims;
1. Reduced morbidity, mortality, and socio-economic consequences.
2. Reduced resources for response and recovery (finance, human and other).
3. Reduced time to implement full response.
4. Reduced length of emergency.

General healthcare dispensation is founded on the critical applications of defense in depth macrocosm with an objective to;
1. defend or delay populations from getting diseases,
2. fight-off existing diseases together with their complications,
3. And to recover or remobilize resources to maintain optimum levels of health and wellbeing of a population within a given national boundary.

Objectives:
1. To develop Lesotho’s healthcare system to be dynamic, adaptable and dependable as a tool or system of healthcare.
2. To develop Lesotho’s healthcare equity in the direction of universal healthcare coverage with reproducible results.
3. To inform and transform Lesotho’s healthcare interventions in accordance with regional population health management needs whilst aware of global healthcare research development.

Myasthenia gravis (MG)

MG is a rare, acquired, autoimmune disorder caused by an antibody-mediated blockade of neuromuscular transmission resulting in skeletal muscle weakness and rapid muscle fatigue.

MG presents with painless, specific muscle weakness and not generalized fatigue.

The clinical hallmark of MG is the presence of oscillating fatigable muscle weakness that worsen with activity and improves on rest.

85% of MG present with ocular muscle weakness symptoms with or without generalized weakness.

Why Primary healthcare approach?
Response: Primary health care approach is viewed as a smarter, evidence-based healthcare advance with a hybrid, empirical origin that has become an integrated event tailored to meet the demands of evolving health practice and patient centered care.

PHC and UHC are both viewed as a political choice for any country to achieve “health for all and all for health” developmental milestones in proposed healthcare related SDGs.

PHC and UHC have been proven to be relevant as global monitors of health services provision. The outcomes are that it works, that it doesn’t work without political support or leadership and that it has to be integrated within existing systems of healthcare to be optimally functional.

PHC is viewed as the backbone of health systems within a country. Covid-19 pandemic and other previous pandemics have explicitly laid bare evidence that PHC functions as the gateway into health-care services, an atypical triage where rapid screening, reporting, monitoring & evaluation, management and referrals are made within a single system.
