A middle aged lady with newly diagnosed hypertension was referred by my colleague for cardiac assessment when she presented initially with giddiness sounding like vertigo and occasional non-specific palpitations.

No shortness of breath, No chest pain, No typical symptoms of angina.

ECG and Echocardiogram were completely normal.

Decided to work her up for coronary artery disease due to her risk factors.

CTCA showed moderate disease and proceeded with angiogram which noted so severe LAD disease.

Subsequent angioplasty was done and she went home feeling great and thankful that early detection of significant coronary artery disease made a difference in her life.

📝Moral of the story: Awareness is the key to early detection of coronary artery disease.

The year 2025 has been a truly remarkable year for our Cath Lab. Since the Cath Lab began its operations on 7th February 2025 with the introduction of the new biplane system, our newly assembled Cath Lab team comprising of mixture of senior and junior staff had demonstrated outstanding commitment, professionalism, and teamwork right from day one.

Over the past 11 months, we have successfully performed around 550 cases, ranging from routine procedures to complex and high-risk cases (CHIP).

These include:
• 24-hour Primary PCI (day and night)
• Electrophysiology cases
• Pulmonary thrombectomy
• Peripheral (lower limb) cases
• Interventional radiology cases

Each case is not merely about the procedure, but involves:
• Careful preparation
• Strong team coordination
• A high level of focus on patient safety
• Prompt action during emergency situations

All of this is made possible through the hard work, discipline, and dedication of the entire Cath Lab team.

I would like to express my deepest gratitude to the entire team （CVTs, nurses, radiographers, and support staff）for your unwavering support, professionalism, commitment and for always placing patients as your top priority.

Not everyone will notice the effort and sacrifice made by the Cath Lab team - handling cases outside office hours, long complex cases, and even emergencies.

You are the backbone of this Cathlab.

Every patient treated safely and successfully is a result of our collective teamwork.

2025 年，对我们的心导管室（Cathlab）而言，是意义非凡的一年。

自 2025 年 2 月 7 日心导管室正式启用，并引入全新的双平面（biplane）系统以来，团队中的每一位成员从第一天起，便展现出令人敬佩的投入、专业精神与高度的团队合作。

在约11 个月的时间里，我们携手完成了约550 宗病例，涵盖从常规手术到复杂且高风险的病例（CHIP）。

包括但不限于：
• 全天候 Primary PCI
• 心脏电生理检测与治疗
• 肺动脉血栓切除术
• 外周血管介入手术（下肢）
• 介入放射学相关病例

每一宗病例不仅仅是一项手术，更代表着：
周密而严谨的术前准备、
紧密而高效的团队协调、
对病人安全始终如一的高度重视，
以及在紧急情况下迅速、果断的应对能力。

这一切的成果，皆源自于大家的努力、纪律与无私的奉献。

Pulmonary Embolism Can Be Life-Threatening 🫁

Blood clots can form spontaneously in the pulmonary arteries (major arteries that supply blood from the lungs to the heart) leading to a life-threatening condition known as pulmonary embolism and may cause sudden death if not treated promptly.

A man in his 40s, previously healthy, presented with shortness of breath, cough, and fever after returning from overseas travel. Initially thought to be a simple lung infection but further tests done revealed massive pulmonary embolism, with large blood clots in both main pulmonary arteries.

Together with Dr. Kurnia Prima Sawai, we performed a percutaneous pulmonary embolectomy using the FlowTriever device, successfully removing the blood clots from the pulmonary arteries. The patient improved well after treatment.

👉 Bagan Specialist Centre is the first hospital in mainland Penang and the northern states to use the FlowTriever aspiration system for massive pulmonary embolism — a less invasive method of embolectomy alternative to open surgery.

Warning signs of pulmonary embolism:
⚠️ Sudden shortness of breath
⚠️ Chest pain
⚠️ Cough (sometimes with blood)
⚠️ Fast heartbeat or fainting

🫀 Not all breathlessness and fever are due to infection.
Please seek medical attention immediately if these symptoms occur, especially after long-distance travel.

肺栓塞可危及生命 🫁

肺动脉血栓（肺栓塞）是一种可致命的急症，若未及时治疗，可能突然夺命。

一名 40 多岁的健康男性，海外旅行后出现呼吸困难、咳嗽及发烧。初步看似肺部感染，但进一步检查证实为肺栓塞，双侧主肺动脉内有大量血栓。

我与Dr. Kurnia Prima Sawai使用 FlowTriever 装置成功进行肺动脉血栓清除术，患者术后明显好转。

👉 Bagan Specialist Centre 为威省及北马地区首家使用 FlowTriever 抽吸系统治疗肺栓塞的医院，这个治疗方式相比传统手术创伤较小，好让病人可以恢复得更快。

肺栓塞警示症状：
⚠️ 突然呼吸困难
⚠️ 胸痛
⚠️ 咳嗽（可能咳血）
⚠️ 心跳加快或晕厥

🫀 并非所有气喘与发烧都是源自细菌/病毒感染。
若出现异常症状，尤其长途旅行后，请立即就医。
