Pei Gin is a regular personal training (PT) client. She often attends our regular evening classes at 830pm on Tuesday, Thursday, and Saturday. Yesterday, she suffered from severe headache that started on the left frontal lobe which spread to her whole head. She also started to feel nausea. As she cancelled the usual group training, I invited her to volunteer for a demo session to show some of my thought process in how I treat my clients.

I intended to bring her center of mass (COM) back to the middle as she was pushed forwards significantly. This shift of balance forwards creates tension in the back and neck which may lead to headaches and nausea. We started by mobilising the shoulder blades, rotating the thorax (T1-12), a bit of expanding the chest, and releasing some tension on the back of her head with a quick deep massage. Hopefully you can see in the video of her COM coming back to the middle, allowing her to stand taller with more freedom. As a result, she had almost zero headache and zero nausea within 15 minutes of work. In a typical 45 minute session, we would have continued to expand the chest further, release more neck and head tension, and provide a better connection of the hip and pelvis to the head.

LOWER BACK PAIN - BALANCE
Lower back pain is one of most common causes of poor quality of life in all ages. There are a myriad of causes for lower back pain including degenerate lumbar disk, a pinched nerve usually resulting in sciatica, slipped disk, and more. Although there are many different types, one can simplify these different circumstances into either a lack of ability to compress and expand properly in the pelvis, ribcage, or both.

At the level of the pelvis, individuals usually lack both or mostly compression relative to expansion. At the level of the ribcage, most are compressed at the areas of T7-12 as well as the sternum at the front and lack expansion. This often creates a “way back posture” where the pelvis falls forwards, collapsed sternum, chin forwards, and an overall shift of balance to the front.

This shift in balance creates an overall increase in tension through the whole system and will cause pains and aches in many areas of the body. One of these many areas is commonly the lower back. What we want to do in this scenario to “fix” lower back pain is by bringing back some compressive abilities of the pelvis and hips while also expanding T7-12 and the sternum. This will shift the overall balance back to the middle and take pressure off of the lower back reducing the cause of the pain and the pain itself.

Are there good or bad postures? Are these categorisation of posture accurate?

Most talk about posture as a static position for any given time. Unfortunately, posture is NOT a static but rather an active position within a specific timeframe. “Good” posture reflects an ADAPTABLE BODY to different situations. “Bad” posture reflects the INABILITY to do so.

An easy way to understand this concept is to pay attention to sports. It is very rare to see an athlete with a stiff and erect spine while running, changing directions, taking or giving contact, throwing, jumping, or any athletic movements. In most, if not all sports, the athlete must be able to change the positions of different parts of the body to change direction aggressively, perform a high jump over a pole, and so on. On top of that, not all athletes stand with a full erect spine all the time. Arguably the best swimmer of all-times, Michael Phelps, has very rounded shoulders and flexed thoracic ribcage as his default while still having healthy shoulders and obviously great performance. When you look at athletes, the idea we use as “good” posture, simply doesn’t apply.

“Good” posture is determined by our ability to adapt to different situations as easily as possible with the greatest amount of results, which defines efficiency. There is no such thing as “bad” posture. There is only lack of movement capacity, leading to overuse or major injuries. In order for one to have “good” posture, one must learn to increase the movement variability in the body so we can better adapt to different situations at any given moment.

How do we achieve this capacity for variation? In our practice, we start by exploring the movement capacity that is already there and enhance on it with easy and gentle movements. As the brain recognises comfort and ease in them, options and variations will open up, which we will continue to explore gently. Over a period of time and repetition, the body will be exposed to more variation of movements and increase its capacity to move. Posture becomes more adaptable as whole and overall well-being improves. In most cases, this also results in less pain in any particular area of the body.
