40-year-old gentleman with long-standing, treatment-resistant hypertension. Routine labs revealed persistent hypokalaemia, raising suspicion for a secondary cause. Further endocrinological workup confirmed primary hyperaldosteronism, and imaging identified a left-sided adrenal mass—likely an aldosterone-producing adenoma. This case highlights the importance of considering endocrine causes in younger patients with difficult-to-control blood pressure.
