A. GENERAL CARE : SKILLED NURSING & ASSISTED LIVING
Medically necessary and daily life assistance services from our lovers who are professional nurses, medical assistants, physiotherapists and nursing aides.
24 hours Nursing Care & Supervision: 24 hours 7 days nursing care and supervisions from lovers on duty at all times.
Short Term & Long Term Residential Care: Short term and long-term care for elders or patients who face difficulties living at their home due to illness, and disability.
Continuous Assessment & Monitoring: Ongoing nursing assessment and monitoring for improving patients' health condition and their stay at the campus.
Activity of Daily Living (ADL) Assistance: Six basics ADLs assistance provided which are ; eating, bathing, getting dressed, toileting, transferring and continence.
Visit by Consultant Physician: Regulatory visits (upon request) by doctors to address patient medical needs.
Respite Care: Short term, time-limited breaks or an emergency temporary care to primary caregivers from care-giving. We understand that sometimes you need a break from care-giving your love ones.
Medication Administration: Preparing, giving and evaluating the effectiveness of prescription and non-prescription medicine to the patients.
Pain Management: Medical treatment and care for pain includes medications, therapies and mind-body techniques.
Personal Hygiene Management: Proper personal hygiene management to maintain the cleanliness of patients because it is is one of the essential parts of daily life.
Toilet Training : Toilet training skills for patients especially dementia who experience difficulties using toilet.
B. SPECIFIC CARE PLANS FOR PATIENTS
Specific care plans tailored to meet each and every patient's special needs as recommended by consultant physician.
Care for Dementia : Specialist care for people diagnosed with dementia.
Physical Disability Support: Personalised care with equipment to aid physical disabilities. We also provide reablement program which is a short term support to help patient prepare to live independently at home following an illness or operation.
Post Stroke Nursing Care : Comprehensive nursing care plan for post-discharge stroke patient in order to minimise and manage complication, to increase patient's accountability and to educate patient's family or primary caregiver.
Brain Injury Care: Care and rehabilitation for brain injury patient to improve abilities to perform daily activities.
Ventilator Patient Care: Care and supervision on patient who depends on machine o support breathing.
Fracture Care Treatment & Management : Medical treatment and nursing care for broken bones and joint dislocations due to weak bones, car accidents, falls or sports injuries.
Palliative Care: Care given to improve the quality of life of patient with life-threatening disease, such as cancer. We provide comfort & supportive and symptom management that addresses the person as a whole, not just the disease.
Spinal Care : Chiropractic service and massage therapy for spinal care including back pain, neck pain, headaches and other chronic pain.
Post Surgical Care: Care provided for patient after operation procedure depends on the type of surgery and health history.
Diabetes Care & Management: Care for diabetic patient including medication, diabetic meal plan, exercise and blood sugar level monitoring.
Tracheostomy Care: Tracheostomy care and tracheal suctioning for patient with an opening in the neck (the breathing tube) to help breathing.
Stoma Care Nursing Management: Care provided for patient with a surgically opening in belly's wall in order to pass waste (faecal matter or urine) from the body.
Care for Bedridden Patient: Full time effective care for bedridden patient to prevent and to manage complications such as bedsores, pneumonia, constipation, contractures, deformity and stiffness, Recurrent Urinary Tract Infection and depression.
Nasogastric Tube Management & Care: Care provided for patient who depends on medical feeding tube for food and medicine.
Urinary Catheter & Suprapubic Catheter Care: Care provided for patient (male and female) who depend on a rubber drainage tube to urinate.
Amputation and Stump Care & Treatment : Care (including dressing care, wound care) and rehabilitation for patient after amputation.
Skin & Wound Care: Care and treatment for skin and wound including basic care dressings and bandages, diabetic wound, traumatic wound (accident) and bedsores.
C. PHYSIOTHERAPY & REHABILITATION PROGRAM
Our physiotherapists (the lovers) will provide physiotherapy service and rehabilitation program by assess, diagnose & treat health conditions, prevent movement disorders and help to repair damage, reduce stiffness and pain, increase mobility and improve quality of patient's daily life.
General Rehabilitation : The goal of general rehabilitation is long-term self-management and independence. Our treatment plans address mobility, self-care, cognitive abilities (memory, understanding and communication deficits) and endurance for maximum participation in healthy daily living.
Stroke Rehabilitation : To help stoke patient to relearn skills that lost when a stroke affected the part of the brain, to help regain independence and to improve quality of life.
Spinal Rehabilitation : To help spinal patient by emphasizing maintenance and strengthening of existing muscle function , redeveloping fine motor skills and learning adaptive techniques to accomplish daily tasks.
Brain Injury Rehabilitation : To help the brain learn alternative ways of working in order to minimise the long-term impact of the brain injury. To help the patient and the family to cope successfully with any remaining disabilities.
Occupational Therapy: To help patients to do the things they want and need to do through the therapeutic use of daily activities (occupations).
Speech & Swallowing Therapy: Our lovers will evaluate the specific needs depend on the condition of the patient, and suggest treatments for articulation, language, stuttering, voice, cognition and swallowing disorders.
Cognitive Stimulation Therapy: A program of themed activities led by lovers. Each session covers a different topic and is designed to improve the mental abilities and memory.
Massage Therapy: Massage therapy for health purposes and given depends on the needs and physical condition of the patient.
D. ADDITIONAL SERVICES (available upon request)
Private Nurse: Our Clearwater Care Lovers provide private duty care work one-on-one with individual patients.

Acupunture : Chinese technique and treatment to bring the body back into balance, healing patterns of disease, increase the immune response and improve energy levels.
Aromatherapy : Provide healing energy of aromatherapy with special blended oil to improve patients' overall wellness.
Transportation : Medical transportation by private ambulance to transport patients to doctor appointments, to and fro hospital or home.
