An anterior open bite is a type of malocclusion where the upper and lower front teeth do not touch when the back teeth are together, leaving a vertical gap between them.

🦷 Types of Anterior Open Bite

1. Dental open bite

-Caused primarily by the position of teeth.
-Teeth fail to erupt properly in the anterior region.
-Often linked to habits like thumb sucking or tongue thrusting.

2. Skeletal open bite

-Caused by jaw growth discrepancies.
-The upper and/or lower jaw grows in a way that prevents front teeth from contacting.
-Often associated with long face syndrome.

⚠ Causes

-Oral habits: thumb sucking, pacifier use, tongue thrusting.
-Skeletal growth problems: excessive vertical growth of the jaw.
-Genetic factors.
-Airway issues: mouth breathing leading to altered jaw posture.
-Temporomandibular joint problems or trauma.

🔍 Clinical Features

-Visible gap between upper and lower front teeth when back teeth are together.
-Difficulty biting into foods like sandwiches or apples.
-Speech problems (lisping or improper tongue placement).
-Aesthetic concerns (long facial appearance or “gummy” smile)

What is a Protraction Facemask?

The device commonly referred to as a protraction facemask (also called a reverse-pull headgear or orthodontic facemask) is an extra-oral orthodontic appliance used primarily in growing children.

Purpose

It’s used when the upper jaw (maxilla) is under-developed or positioned too far back relative to the lower jaw (mandible) — essentially in a skeletal Class III malocclusion.

The facemask applies forward and downward traction on the maxilla (via elastics hooked to a fixed intra-oral appliance) to encourage maxillary advancement.

By doing this early, the hope is to reduce the severity of the discrepancy and possibly reduce the need for more invasive treatment (e.g., jaw surgery) later.

What it Can (and Cannot) Do

✅ Potential Benefits

Can advance the maxilla forward (modestly) and help correct or reduce the underbite/crossbite.

Improve dental occlusion (overjet, overbite) in growing patients.

Possibly reduce the extent of more invasive treatment later (e.g., surgery) in some cases

Anterior crossbite is a dental condition where one or more of the upper front teeth (incisors) bite behind the lower front teeth instead of in front of them when you close your mouth.

Here’s a quick breakdown 👇

🦷 Types of Anterior Crossbite

1. Dental crossbite – caused by the position of the teeth (misalignment or crowding).
2. Skeletal crossbite – caused by jaw bone structure, such as when the lower jaw (mandible) is too far forward or the upper jaw (maxilla) is too far back.

⚠ Possible Causes

-Delayed loss of baby teeth
-Early eruption of permanent teeth
Habits (like thumb sucking or tongue thrusting)
-Jaw growth discrepancies (skeletal Class III pattern)
-Trauma to primary teeth

🧠 Why It’s Important to Treat
If left untreated, it can cause:

1. Uneven tooth wear
2. Gum recession
3. Aesthetic concerns
4. Functional problems (difficulty biting or chewing)
5. Jaw growth imbalance

✨ Class III (Underbite) malocclusion
Before-and-After Comparison ✨

This case shows more than just a smile transformation — it reflects a significant improvement in facial balance and jaw harmony.

This was a mild to moderate Class III malocclusion case.
With careful orthodontic planning, we were able to achieve this result without the need for jaw surgery — demonstrating how even non-surgical treatment can bring significant improvement in both bite and facial profile.
✅ Before treatment: Notice the prominent lower jaw and retrusive upper jaw, typical in skeletal Class III cases.
✅ After braces: The profile now appears more proportionate, straighter facial profile, good alignment with occlusion. The improved jaw alignment gives him a softer look and a more friendly appearance.

🦷 Braces don’t just align teeth; they help harmonize the teeth, jaws, and overall facial aesthetics.

Duration for this case completion- 26 months

Oral Myofunctional Theraphy (OMT)

OMT is specialized treatment that treat Orofacial Myofunctional Disorders (OMD).
OMT focuses on retraining the muscles of the face and mouth to improve function and balance, particularly related to breathing, chewing, swallowing and speaking.
What are Orofacial Myofunctional Disorders?
In order to properly speak, breath, and swallow, the muscles in our face, mouth, and throat need to be structured in the correct position and coordinate as they move together.
Orofacial myofunctional disorders (OMDs), according to the American Speech-Language and Hearing Association, are “patterns involving oral and orofacial musculature that interfere with normal growth, development, or function of orofacial structures, or call attention to themselves.”
Here are a few examples of Orofacial Myofunctional Disorders:
Tongue thrust (tongue between the teeth while swallowing)
Mouth breathing (lips apart/mouth open at rest)
Jaw malocclusions (such as an overbite, underbite, or open bite)
Tongue tie
Lip tie
OMDs can be caused by a combination of a physical difference in the structure of the mouth/facial muscles and patterns of behaviors over time.
Causes include:
Genetics
Orthodontic issues
Thumb or finger sucking
Prolonged use of sippy cups/pacifiers
Structural abnormalities
Neurological disorders
Obstructed nasal passages (for example, due to enlarged tonsils or adenoids, or allergies), causing the tongue to be in an improper position.
Signs of Orofacial Myofunctional Disorders
What are some signs that a child may have an Orofacial Myofunctional Disorder?
He or she may have trouble with speech, swallowing, or breathing. More specifically, some signs and symptoms of OMDs include:
Speech articulation difficulties (such as lisp, where the child produces the “S” and “Z” sounds with his or her tongue forward and between the teeth).
Drooling or an open-mouth posture.
Disordered sleep (such as Obstructive Sleep Apnea) or sleep disordered breathing (such as snoring).
Difficulty swallowing with the lips closed.

An impacted lower second molar is a tooth that doesn't erupt properly through the gums because it's blocked by other teeth, bone, or soft tissue. This impaction can occur in the mandible (lower jaw) and can be unilateral (one side) or bilateral (both sides).

This can lead to various problems, including tooth decay, gum disease, and even damage to neighboring teeth. The primary problem with an impacted lower second molar is its potential to cause crowding, malocclusion, and other orthodontic issues.

Treatment options for impacted second molars include orthodontic correction, surgical removal, or other specialized techniques.

In this case, the right impacted lower second molar was uprighted with orthodontic (braces) treatment.

Does my child need a facemask? Understanding Early Orthopedic Treatment
As children grow, their facial bones are developing too — and sometimes, they may need a little guidance to ensure everything aligns properly. One common tool in orthodontics for growing kids is the facemask, also known as a reverse-pull headgear.

What Is a Facemask?
A facemask is a removable orthopedic appliance used to correct underdeveloped upper jaws (maxilla). It gently pulls the upper jaw forward using elastics connected to braces or an expander inside the mouth.

Who Needs It?
Children with Class III malocclusion (commonly known as an underbite) often benefit from early facemask therapy. The earlier it's used, the better the results — because the bones are still flexible and growing.

Why Early Treatment Matters:
• Guides proper jaw growth
• Reduces the need for jaw surgery later
• Improves bite and facial profile
• Enhances confidence and speech

Is It Painful?
It's not painful, but it can take some getting used to. Consistent wear (usually 12–14 hours a day) is key to seeing progress.
