Here is the explanation:
Pretibial myxedema occurs due to autoimmune stimulation of fibroblasts by thyroid-stimulating immunoglobulins, the same antibodies that cause the hyperthyroidism and eye signs in Graves!

These fibroblasts overproduce
glycosaminoglycans, leading to the thickened, waxy, orange-peel appearance of the skin over the shins.

The process is localised, painless, and often occurs alongside ophthalmopathy in patients with established Graves' disease.
