Following the first successful slide tracheoplasty performed in a Malaysian private hospital for a child with congenital tracheal narrowing, Pantai Hospital Kuala Lumpur continues to advance paediatric airway care and share expertise regionally.

The milestone surgery which involved repairing both the trachea and heart in a single operation for a one-year-old child with complex airway and congenital heart conditions — reflects the level of specialised, multidisciplinary care available at PHKL.

Building on this success, PHKL hosted Malaysia’s first private-hospital Slide Tracheoplasty Workshop, led by Consultant Paediatric Cardiothoracic Surgeon, Dr Siti Laura.

The hands-on workshop welcomed 6 ENT and Cardiothoracic Surgeons, including participants from Sabah and Jakarta, Indonesia. Using animal trachea models and scope-based learning, participants gained exposure to critical surgical techniques and real-world clinical decision-making.

This pioneering initiative underscores PHKL’s commitment to advancing paediatric airway care through specialised expertise, multidisciplinary collaboration, and regional knowledge-sharing.

This one-year-old baby was born with a narrowed airway, a condition that becomes more severe as the child grows. The narrowing made it increasingly difficult to breathe and oxygenate well, causing noisy breathing and visible effort just to get enough air. Feeding was also a challenge, and at one year old, the child weighed only 6kg.

For many children with this condition, prolonged stays in the NICU are common, sometimes lasting months. When the narrowing occurs within the chest (intrathoracic), as in this case, the condition is even more complex. Surgery becomes the only option.

Led by Consultant Paediatric Cardiothoracic Surgeon Dr Siti Laura and Consultant ENT Specialist Dr Rohaizam Bin Japar, our multidisciplinary team took on this complex challenge. The airway narrowing occurred alongside congenital heart disease, a combination seen in many children with this condition. What made this surgery remarkable was that both the trachea and the heart were repaired in a single operation, requiring precise coordination across surgical, cardiac, anaesthetic, ICU and OT teams.

The procedure was supported by Consultant Anaesthesiologists Dr Hamidah and Dr Sivaraj Chandran, whose expertise was critical to the success of this highly complex surgery. We also extend our appreciation to the OT and ICU teams for their exceptional care before, during and after the procedure.

Following surgery, a scope examination confirmed a well-repaired airway. Today, the child has recovered well and has returned home, a powerful reminder of what is possible when expertise, teamwork and compassion come together for even the smallest patients.
