This case report describes an adult patient with long-standing childhood strabismus who developed ocular torticollis, facial asymmetry, and secondary spinal changes as a result of prolonged visual compensation. The visual etiology remained unrecognized until adulthood, after many years of musculoskeletal-focused management. Following diagnosis, intensive neuro-vision therapy led to improved ocular alignment, the development of depth perception, and a marked reduction in abnormal head posture. These outcomes demonstrate that visual rehabilitation can be effective even later in life. This case highlights the critical importance of early ophthalmic evaluation and a multidisciplinary management approach involving both an ophthalmologist and a behavioral optometrist.
