PRP (platelet-rich plasma) is prepared from a small sample of a womanโ€™s own blood and contains multiple growth factors that support healing and tissue health.

Ovarian PRP may help improve ovarian activity. It is being explored especially for women with low ovarian reserve, reduced ovarian function and premature ovarian failure.

Endometrial PRP is used to support the lining of the uterus. It may help the uterine lining become thicker and healthier, which is important for embryo implantation. It's role in thin endometrium and recurrent implantation failure is still being investigated.

While research is still ongoing, and results or outcomes are not guaranteed, PRP is seen as a promising and hopeful option for some patients facing fertility challenges.

Not everyone with infertility needs fertility treatment. This couple has been married since 2021. Wife is now 40 years old.

A scan with me at Sunfert@Bangsar South showed what looked like endometrial polyps in Mum's uterus. True enough, a hysteroscopic examination found polyps which were removed and, an abnormal endometrial lining which required prolonged progesterone treatment to correct it. This took several months, and a subsequent repeat hysteroscopic examination showed a normal uterine lining.

However, just 2 months after completion of her treatment for disordered endometrium, while we were preparing Mum for her IVF and frozen embryo transfer, she conceived spontaneously, all on her own ๐Ÿ˜„! And, it wasn't with just 1 baby, but with twins ๐Ÿ‘ถ ๐Ÿ‘ถ !!!

Their due date is in August 2026. Hope all goes well for them ๐Ÿ˜ƒ. Tahniah, Ibu dan Ayah.

An isthmocele (also called a cesarean scar defect or uterine niche) is a small pouch or indentation that can form in the wall of the uterus, at the site of a previous caesarean section scar. It happens when the old scar doesnโ€™t heal completely.

An isthmocele can impact IVF in several ways:
1. Fluid in the uterus - the pouch can collect blood or fluid, which may leak into the uterine cavity. This fluid can interfere with embryo implantation.

2. Inflammation
Trapped fluid or old blood can cause chronic inflammation of the uterine lining. Inflammation may reduce endometrial receptivity and impair embryo implantation.

Does an isthmocele always need treatment before IVF? It depends on the size and depth of the isthmocele and, whether fluid is seen within the uterine cavity.

Treatment may be considered if:
a. There is persistent uterine fluid during IVF cycles
b. There have been repeated implantation failures
c. The defect is large or symptomatic

So, do I ever take credit for someone else's work? Well yes, it has occurred before ๐Ÿคฃ.This commonly happens when couples move their frozen embryos from their previous clinic, over to us at Sunfert@Bangsar South, and we manage to get them pregnant. And then, we're the ones who get rewarded with yummy Christmas themed brownies ๐Ÿ˜!

โ˜† This couple did 18 clomid/letrozole ovulation induction cycles over 2 years, in their previous centre. All failed.
โ˜† They then proceeded with an IVF cycle with their previous Doctor, in 2024. They made 2 euploid blastocysts.
โ˜† Their first frozen embryo transfer with a euploid embryo failed.
โ˜† They then moved their remaining euploid embryo to me at Sunfert@Bangsar South.
โ˜† In October 2025, we transferred their euploid embryo with some hormonal manipulation and immune modifiers thrown in. And it worked! They're now 12 weeks pregnant โค๏ธ!

This couple is extremely grateful to us at Sunfert@Bangsar South. However, I did remind them that the embryo wasn't even ours to begin with, so, some credit still goes to their previous clinic ๐Ÿ™.

Their due date is in July 2026. Congratulations Mum and Dad. Have a safe pregnancy and delivery.

What do you do when all your euploid embryos fail to implant? Well, there's some evidence that suggests we don't need to do anything differently, just keep transferring the remaining embryos, one will eventually stick.

Did I do that? Absolutely NOT ๐Ÿ˜‚! Anyone who knows me, would know that I'd have ordered a string of tests for both, husband and wife.

And, we identified a few issues with:
1. Dad's s***m, minor issues
2. Mum's clotting factors
3. Mum's immune factors
4. Delayed endometrial receptivity window
5. Sub-optimal microorganism milieu in Mum's uterus

Here's their story:
โ–  Their 1st IVF in 2023 was at their previous centre, and they made 2 beautiful PGTA tested euploid embryos. Unfortunately both embryos failed to implant in 2 separate frozen embryo transfer (FET) cycles.

They then came over to see me at Sunfert@Bangsar South.

โ–  Their 2nd IVF was in June 2024. They decided to forgo PGTA testing this time round but agreed to some other tests that I wanted to carry out.

These results allowed us to modify their s***m selection technique in our embryology lab and, we added some extra culture steps for their embryos. Again, I remind you, the evidence for these techniques are not strong. Some might even call these 'tricks' of mine 'witchcraft' ๐Ÿˆโ€โฌ›๏ธ ๐Ÿˆ ๐Ÿˆโ€โฌ›๏ธ. Heck, I call it 'witchcraft' as well ๐Ÿงน๐Ÿง™โ€โ™€๏ธ๐Ÿ˜‚!

โ–  Their 1st FET at Sunfert@Bangsar South was in September 2024, with a non-PGTA tested, 3BB embryo. (Spoiler alert: Mum's already delivered the cutest baby boy ever, in May 2025)

This was the 'witchcraft' I tried in her 1st FET cycle with me:
ใ€‹I added some hormonal adjustments
ใ€‹I included some mild immune manipulation
ใ€‹She agreed to anti-clotting factor treatment
ใ€‹We corrected the imbalance in her microorganism milieu
ใ€‹We dragged out her FET date to fit her implantation window

So, why do I keep practicing my brand of 'witchcraft' despite a lack of strong evidence? The answer is in this lovely picture: Another beautiful family completed!
