In Asia we are seeing an epidemic of diabetes!
Patients often comes with long diffuse coronary artery disease which are challenging to treat

Today with drug coated balloons, this technology offers a unique solution especially in distal small vessels
This allow us to treat such patients without using a stent, potential late lumen gain rather than late lumen loss while preserving the vessel for future option of bypass surgery

This 29 year old gentleman presented with heart failure
He has poorly controlled diabetes
The LAD is diffusely diseased

Here we took a hybrid approach by treating the proximal lesion with bioadaptor DynamX and the distal vessel with Restore Plus DCB
The introduction of 40mm length is ideal in this scenario

Back to work after the lunar new year celebrations
Straight to the cathlab

This 36 year old man came with symptoms of angina
He has diabetes diagnosed 5 years ago which is poorly controlled
A CT coronary was performed which confirmed severe disease proximal and distal LAD

Coronary angiogram with OCT imaging confirmed absence of calcific plaque
Thus we treated the pLAD with resorbable scaffold Freesolve and dLAD with drug coated balloon

After 1 year, the vessel will be cage free back to it’s original anatomy and physiology allowing it to be vasomotor reactive

These technology allow us to somewhat “reverse Artherosclerosis” hopefully providing more quality years of life for our patients

Laser? Lithotripsy? Sonic Waves? Pulses?

We have the opportunity to treat this heavily calcified vessel with SoniCracker IVL balloon
The device performed well with good cross ability and effective sonic waves to crack the calcified plaque in order to allow ultimately excellent stent expansion

After treating the LAD we completed revascularisation with angioplasty of the LCx chronic total occlusion

Perform this case with our senior consultant Dr Liao Chi Ming at Gleneagles Hospital Kuala Lumpur

I also understand that this is the first time this device is used commercially in a private hospital setting for the central region in Malaysia

Introducing a new service at Gleneagles Hospital Kuala Lumpur

We perform our first catheter pulmonary embolectomy with the Inari Flowtriever system on a patient whom presented with submassive pulmonary embolism

Procedure went well supported by our anaesthetic colleagues Dr Suresh and Dr Paul

This device allow us to treat patients with pulmary embolism without open heart surgery
Pulmonary embolism can often be lethal and difficult to diagnose
A high index of suspicion is required to pick up these cases
Kudos to the team at Gleneagles KL including the emergency department for accurately detecting this diagnosis

Ending the year with a streak of complex cases

One particular case done last Monday was a CTO proximal right coronary artery
It was a long CTO with retrograde from LAD only till distal PD/PL bifurcation

We started with AL 7Fr guide and Fielder XT cross the proximal cap microchannel

We then wire escalated to UB3 and Gaia II which cross to what seem distal RCA

Descalate to Pilot 200 but noted wire is actually in RV branch
Decide to exchange with work horse wire and sequential balloon from RV into proximal RCA

Distal RCA was then seen after ballooning
We then parallel wire with Pilot 200 into distal RCA but was not able to cross distal cap

Escalated again to Gaia II which cross into PL branch

Completed the procedure with DES implantation

Probably one of the most challenging case till date at Gleneagles Hospital Kuala Lumpur

The final shot and a happy patient made the process worthwhile

Last Tuesday we introduced a new Intravascular imaging device in Gleneagles Hospital Kuala Lumpur

This is none other than Insight Lifetech Co., Ltd. Truevision IVUS

Our first case was a lady with diabetes presenting with long diffuse disease of LAD treated with hybrid DES and DCB strategy

After stenting, we notice intramural hematoma at the proximal stent edge extending to the ostial LAD, thus the stent was extended proximally
They IVUS greyscale images are bright and clear as expected with addition of its latest AI tools make decision making quick and easy!
The system has definitely improved since my last experience with it now more than 1 year ago
