Our 3rd paper this year 🎉

ADATPE study
Proud to share our multicentre Malaysian study published in the Journal of Thoracic Disease, evaluating the diagnostic utility of pleural fluid ADA in tuberculous pleural effusion.

In this study, we identified a new local ADA cutoff (≥36 U/L) that improves diagnostic performance and proposed a stepwise diagnostic algorithm integrating ADA with the LDH/ADA ratio to enhance accuracy in pleural TB.

A true collaborative effort across centres in Malaysia — advancing evidence-based care in pleural disease.

Journal presentation by Dr Ooh

Post-tuberculosis lung disease (PTLD) is a silent but significant global burden. Even after cure, many TB survivors develop chronic lung damage, COPD, bronchiectasis, pulmonary hypertension, and higher risks of cardiovascular disease, cancer, and premature death. TB care must go beyond cure — long-term follow-up and integrated management are essential.

CPAP remains the gold standard for obstructive sleep apnea, while HFNC may serve as a viable alternative in selected mild–moderate cases.

Today’s CME by our respiratory fellow critically reviewed the comparative efficacy of CPAP versus HFNC in OSA, focusing on AHI reduction, clinical success rates, and impact across disease severity. The session reinforced the importance of appropriate patient selection and evidence-based treatment decisions in sleep medicine.

Types of Positional Dyspnea — From orthopnea and trepopnea to platypnea and the emerging concept of bendopnea, understanding posture-related breathlessness sharpens our clinical acumen.

Our team organized a CME, we revisited not only the definitions but also the pathophysiological basis, bedside assessment, and clinical implications of each pattern — reinforcing a structured approach to history taking and examination.

Continuous learning, continuous refinement.

Another milestone for our Pleura team — the PENTAD-FT trial now published in BMC Pulmonary Medicine.

A pilot randomized study comparing intrapleural fibrinolytic therapy (tPA + DNase) versus normal saline irrigation in pleural infection — focusing on safety, efficacy, and real-world applicability.

Proud of the entire team for the dedication, discipline, and perseverance behind this work. Small steps forward, but meaningful progress in advancing pleural infection management.
