Congratulations to Dr Nadia on this publication, a collaboration with Faculty of Health Sciences and congrats to the entire team as well. This marks Respiratory HCTM UKM research group’s 5th publication involvement in the first quarter of the year.

This randomized controlled trial showed that low-dose yeast beta-glucan significantly reduced upper respiratory tract symptom severity and improved mood in moderately stressed adults, with the 204 mg dose achieving clinically meaningful benefit and good safety profile.

Encysted lung abscess mimicking a mass lesion. A man in his mid-50s presented with chronic cough. CT thorax demonstrated a localized consolidative mass, initially raising suspicion for malignancy. Image-guided biopsy aspirated frank pus, confirming a localized lung abscess. Follow-up CT after aspiration showed interval reduction in consolidation size with surrounding ground glass, consistent with resolving infection.

Learning Points-
 • Encysted lung abscess can mimic a mass on CT.
• Irregular margins do not always indicate malignancy—consider infection.
• Aspiration of pus confirms diagnosis and avoids misclassification.
• Management differs: antibiotics ± drainage vs oncologic workup.

Diffuse alveolar hemorrhage is a clinicoradiological diagnosis—CT suggests it, but confirmation requires BAL and identification of an underlying cause such as ANCA-associated vasculitis

A woman in her late 60s presented with progressive dyspnea and hemoptysis.

CT thorax demonstrated diffuse bilateral ground-glass opacities with patchy consolidation, in keeping with diffuse alveolar hemorrhage (DAH) in the appropriate clinical context.

She was commenced on intravenous methylprednisolone. Further evaluation revealed positive c-ANCA, and a diagnosis of Granulomatosis with polyangiitis was made.

She was subsequently treated with rituximab and mycophenolate mofetil (MMF)., and able to be extubated.

Follow-up chest radiograph showed interval improvement in pulmonary opacities, consistent with treatment response.

Learning Points – Pulmonary Hemorrhage (DAH)
• CT findings (ground-glass ± consolidation) are suggestive but not diagnostic of DAH
• Bronchoscopy with BAL (progressively bloodier return) supports diagnosis
• Always screen for underlying cause: ANCA, anti-GBM, autoimmune and infection
• Start high-dose steroids early if DAH suspected—do not delay
• Add immunosuppression (rituximab/cyclophosphamide) for vasculitis
• Monitor clinically and radiologically for resolution

Our 3rd paper this year 🎉

ADATPE study
Proud to share our multicentre Malaysian study published in the Journal of Thoracic Disease, evaluating the diagnostic utility of pleural fluid ADA in tuberculous pleural effusion.

In this study, we identified a new local ADA cutoff (≥36 U/L) that improves diagnostic performance and proposed a stepwise diagnostic algorithm integrating ADA with the LDH/ADA ratio to enhance accuracy in pleural TB.

A true collaborative effort across centres in Malaysia — advancing evidence-based care in pleural disease.
