Petang ni aku buat scar revision.

Parut dah jadi widened & depressed. Patient dah try CO2 laser, subcision, microneedling but improvement limited.

At this stage, this is a structural defect

The only definitive option is surgical revision but not simple excision.

So aku guna W-plasty:
• Break the straight line
• Redistribute tension
• Make scar less noticeable

Setiap angle kena precise - planning & ex*****on

Concept nampak simple, but ex*****on actually complex.

End goal: bukan perfect skin, tapi scar yang less obvious & more acceptable long term.

Windscreen injury.
Not just cuts but shattered glass everywhere.

Face, scalp, limbs.
Severe tissue loss. Exposed skull.

3 hours overnight - exploring wounds, removing glass, reconstructing.
Finished at 2 AM.

But here’s the reality…
not all glass can be found during surgery.

Some fragments stay hidden.
They may surface later, form a lump (normal foreign body reaction),
or only show up on imaging.

That’s why this must be explained clearly from the start.

Explain early. Explain clearly.
To avoid misunderstanding & potential medicolegal issues later.

Kes malam ini - dog bite wound

“The solution to pollution is dilution.”

Maksudnya bila luka kotor atau tercemar, yang paling penting ialah basuh luka betul-betul dulu.

Irrigation yang banyak selalunya lebih penting daripada stitch itu sendiri.

Lepas luka dah bersih, barulah kita decide sama ada perlu jahit atau tidak.

Luka nampak kecil, tapi bite wound memang tak boleh ambil ringan.

From today’s OT at Sunway Penang. Managed a case of extensive extramammary Paget’s disease involving the ge***al region. The urologist performed a wide local excision, which left a defect of about 30 × 15 cm. Reconstruction was done using multiple local flaps together with skin grafting. Always thankful for good teamwork.

2 minggu lepas ada discussion antara General Surgery, ENT, Ophthalmology & Plastic Surgery tentang kawasan trauma yang biasanya diuruskan oleh setiap specialty.

Plastic Surgery sebenarnya tak ada kawasan khusus sebab ia cover hampir semua permukaan badan, terutama bila melibatkan pembaikan fungsi & rupa selepas trauma.

Guideline berjaya dibentuk untuk mudahkan kerjasama. Tapi akhirnya, keutamaan tetap pada pesakit & pesakit masih ada hak untuk pilih doktor serta specialty yang mereka prefer bila keadaan mengizinkan.

Me: sure, I’ll apply for GL while you have lunch. After that, continue fasting. I’ll arrange your surgery by today.
