Common admissions these days:

🌡️ Influenza tops the list - high fever, cough, cold. Getting tested early and with antivirals (oseltamivir) can help reduce serious complications such as pneumonia in children.

😮‍💨 RSV bronchioliti ~ infants with fever, cough, wheeze and fast breathing. Always seek prompt or immediate medical attention if your baby is breathing rapidly even if still active!

🫁 Pneumonia (lung infection) persistent fever, cough and fast breathing.

🤢 Acute gastroenteritis , food poisoning, vomiting, diarrhea and abdominal pain

🤒 Dengue - A blood test is done if there is a clinical suspicion or persistent fever >3 -4 day

Protect your baby from RSV bronchiolitis. Nirsevimab is currently available in Malaysia

💉 What is Nirsevimab (Beyfortus)?

Nirsevimab is a long-acting monoclonal antibody that helps protect babies from Respiratory Syncytial Virus (RSV) bronchiolitis and pneumonia.

Unlike vaccines — which help the body make its own antibodies —
nirsevimab works by directly giving protective antibodies to the baby.

How long it lasts:
A single injection gives good protection for at least 5-6 months, enough to cover one RSV season and to protect babies at their most vulnerable age.

RSV circulated in Malaysia throughout the year but two peaks are reported : July -August and October to December

👶 Who should get it

Nirsevimab FDA is approved for newborn and children up to 2 years of age who are at risk of severe RSV infection.

😴 Why Sleep Matters for Growing Children 💫

Adam, is 13. He wakes up at 5.30am every weekday to beat traffic to school. After school, he stays back for extra classes, has dinner late when his parents return, then does homework till 11pm — and only sleeps around midnight after some screen time. With only 5-6 hours of sleep everyday, he feels constantly tired, moody, and irritable...A familiar scene in many busy Malaysian families..

🛌 Sleep isn’t just about rest, physical, emotions and mental health — it’s also when vital hormones work their magic. During deep sleep, growth hormone is released — helping children grow, repair tissues, and stay emotionally balanced.

In addition, poor sleep also affects our circadian rhythm and the way our body secretes cortisol (the stress hormone) which can:
⚖️ Interfere with growth hormone release
💢 Affect mood, focus, memory
🍭 Immunity and energy levels

Parents must also be mindful its not just the number of hours of sleep but the quality. If your child snores loudly, sleeps restlessly, or remains tired despite “enough” sleep, conditions like sleep apnoea, enlarged adenoids & tonsils, allergic rhinitis, asthma, thyroid disorders, or obesity could be the cause — and should be checked.

🕒 Recommended sleep hours baru according to ages and individual, but generally recommended below
👶 3–5 years: 10–13 hours (including naps)
🧒 6–12 years: 9–12 hours
🧑 13–18 years: 8–10 hours

✨ Simple steps can make a big difference — keep consistent bedtimes, reduce screen time before bed, and create a calm wind-down routine.

Quality sleep = healthy growth, balanced hormones, sharper focus, and better emotional wellbeing.

Influenza Season — What Parents & Healthcare Providers Should Know

I’ve been seeing many children with influenza lately. Some important reminders ⬇️

🤒 If your child is unwell
Fever + cough/runny nose/sore throat → test early for influenza.

👨‍⚕️ If symptoms are significant and there’s close contact with a confirmed case
Antiviral (oseltamivir – Tamiflu/Fluhalt) can be started even without testing, as tests can sometimes be falsely negative.

💊 Treatment window
Antiviral works best if started within the first 48 hours, but can still help if your child remains febrile after that.
If fever isn’t settling as expected → think about a secondary bacterial infection.

🛡️ Prevention is key
Get the yearly influenza vaccine — it takes about 2-4 weeks to build protection.

Also dont panic! Most healthy children above 5 and adults recover well from influenza. However always best to consult a doctor early to minimise complications. Let’s protect our little ones and those around them this flu season 🌡️💚

Kawasaki disease~ when a child’s fever doesn’t behave like a usual infection…

I recently cared for a child who was admitted with 3 days of high fever and a tender, red neck swelling. He was initially treated for a lymph node infection, but his fever persisted, and by day 5, he developed red eyes and red, cracked lips. To complicate things further, he was also positive for adenovirus. However as these evolving features pointed to Kawasaki disease, he was promptly treated with IV Immunoglobulin (IVIG) followed by a paediatric cardiologist referral and echocardiogram. Fortunately his fever settled within 24 hours after IV Ig with resolution of his liver enzymes, inflammation markers and gradual normalisation of his platelets.

💡 What is Kawasaki Disease?
Kawasaki disease is an inflammatory condition of the blood vessels (vasculitis) that mainly affects young children under 5 years old, and is seen more commonly in Asian populations.

⚠️ Typical features include:
• Persistent fever (≥5 days, or ≥3 days if typical features appear)
• Red eyes without discharge
• Red, cracked lips or “strawberry tongue”
• Rash on the body
• Redness or swelling of hands and feet
• Enlarged lymph node in the neck, usually unilateral

💉 Why early treatment matters
administration of IVIG should be given as soon as possible when the diagnosis is made, ideally within 10 days of fever onset to significantly reduce the risk of heart complications, particularly coronary artery aneurysms.

👶 A diagnostic challenge
Diagnosis can be challenging as Kawasaki can mimic other conditions and not all features might be present initially. Incomplete forms of Kawasaki disease can occur, especially in infants, and may mimic common infections such as group A strep. scarlet fever, adenovirus and cervical lymphadenitis. Sometimes both conditions can coexist — hence persistent fever despite antibiotics should raise suspicion.

Early recognition and referral to a paediatrician or paediatric cardiologist are key to ensuring the best outcomes.

In Malaysia, it’s normal to tapau hot food in plastic bags or reheat meals in plastic containers. But many parents don’t realise this small habit could be introducing hormone disrupting chemicals into their child’s body.

These are called endocrine disruptors, and they can affect growth, puberty, mood, and even weight especially in children whose bodies are still developing.

🥡 Avoid pouring hot food directly into plastic bags
💥 Don’t microwave food in plastic containers
🌬️ Limit artificial fragrances and strong sprays
🧴 Check ingredients in baby lotions and shampoos

Start with small changes. And if something feels “off” and your child is experiencing early puberty, slow growth, constant tiredness, speak to a paediatric endocrinologist.

Happy to have shared my expertise as an invited speaker at the Sarawak Paediatric Endocrine Conference last weekend, hosted at Sarawak General Hospital with my other esteemed paediatric colleagues and friends. My talk focused on:
🔹 Children born small for gestational age (SGA) with no catchup growth
🔹 The consequences of vitamin D deficiency
🔹 Indications & safety of growth hormone treatment

Sarawak holds special memories for me — before resigning when I was still with Ministey of Health KKM at Putrajaya , I used to do yearly visiting paediatric endocrine clinics there, and also covered Sibu, Sarikei, Miri, Kapit and Bintulu hospitals before the pandemic. Sarawak has always stood out to me for its dedicated healthcare professionals and the wonderful, touching patients and families I’ve had the privilege to meet. Even when they had little, they are rich in graciousness and kindness.
