Many wounds failed to heal because the fundamental first step was not done or not done right i.e. sharp debridement.

This 67 years old gentleman had a lesion in between the 4th and 5th toe and developed into a 4mm diameter, 3-4mm deep wound for the past SEVEN YEARS. He had numerous courses of antibiotics from different GPs and had been seeing various specialists for treatments on top of the daily dressing. A classical history for most of my patients. He is not a diabetic; a non smoker and his cholesterol is controlled.

He came to my clinic last week and was well debrided under local anaesthesia. One stitch was given to stop a spot bleeding because he was on Aspirin for his 2 x angioplasty. It wasn’t an easy debridement due to the difficult position of the wound in between the 2 toes and the tight opening of the wound for my surgical instruments to enter. All non-viable tissues and excessive macerated skin were surgically debrided.

The wound bed was red and healthy post debridement. The wound was left to heal and it healed in 3 DAYS TIME with secondary intention. No antibiotic was used in the course of this treatment because there was no sign of infection.

This is a case of a simple 7 YEARS old wound which was easily healed in 3 DAYS in a moist environment by good debridement with a good antimicrobial wound care solution in the absence of antibiotic use.

The wound was simple but the impact was huge especially for a high-ranking government officer then like him and he now is holding some high posts in the private sectors. Had he been treated well 7 years ago, he would have saved all the money spent on medical expenses and time taken off work for the wound dressings in clinics. Without a good wound care today, he will continue to dress his wound and spend his time and money on this wound for many more infinity years to come.

Good wound care does matter. 🌷

When there are signs of infection or the wound is a high risk wound which is proned to infection, iodine is normally a choice of treatment.

A small wound of 3mm in diameter and 1 inch deep is not only a wound for the patient. It is a daily burden for the family members to have to take turn to clean it daily for this 83 year old lady.

Firstly, the family had to drive her from Raub in a one way of 2 hours plus journey from the village to reach my clinic. After each treatment, they have to repeat the same journey, a total of 5 hours plus on the road just for this tiny wound. Secondly, the daily dressing change. Then the nutritional changes to make due to her old age and lack of appetite. And not the least to teach the family the correct dressing and the correct way to apply the dressing.

Her wound started in April. Had been seeing her regular family clinic doctor for almost two month. But no progress made.
Her family then doctor in Raub recommended her to see me in KL, the first appointment was made in early June.

Nothing in her is in her favour for the wound healing, her age, the cold feet with poor pulses despite the fact she is not diabetic, her poor nutritional status, her immobility, the difficulty to access through the narrow 3mm diameter opening to the deeper part of the wound to clean and so on.

Conventional dressing with me had not made any changes on her. We then switched to higher end dressing and the wound started to shallow up despite the diameter of the wound remain the same.

At the end, the tissues continued to grew to the skin level making the 1 inch depth sinus disappeared. And naturally the skin did the final blessing to close it.

The hard work had paid off and I happily sent her off to the door. I advised her to wear socks to protect the feet and to keep warm.
