This suggests that ECPs are not just educators, but trusted authorities whose recommendations directly guide parental acceptance and implementation of control strategies.

76-year-old female with no medical history reported.
1.Visual Acuity
Right Eye: 6/60 unaided → 6/9 corrected
Left Eye: 6/60 unaided → 6/9 corrected

2.Intraocular Pressure
RE: 18 mmHg
LE: 18 mmHg

3.Key Findings
a.Bilateral cataract
b.Hazy fundus view, likely due to cataract
c.High cup-to-disc ratio (CDR) in both eyes
→ Glaucoma suspect

4.Management Plan
Refer for cataract surgery evaluation and Full glaucoma assessment.

Summary
This 76-year-old female presented with bilateral cataracts and a high cup–disc ratio, raising suspicion of glaucoma. While vision improved with correction, the hazy fundus view limited accurate optic nerve assessment. Normal IOP does not exclude glaucoma, especially in elderly patients. The patient requires cataract surgery evaluation and a comprehensive glaucoma work-up to confirm diagnosis and prevent potential progression to irreversible optic nerve damage.

a. 双眼白内障
b. 眼底影像模糊，可能由白内障引起
c. 双眼视杯／视盘比偏高
→ 青光眼怀疑

4. 管理计划（Management Plan）
建议转介进行白内障手术评估和完整的青光眼检查

这位76岁的女性患者同时存在双眼白内障，并在检查中发现视杯/视盘比增大，提示可能患有青光眼。虽然矫正视力有所改善，但由于白内障导致眼底影像模糊，视神经的评估受到限制。正常眼压并不能排除青光眼，尤其是在老年患者中。患者需要接受白内障手术的评估，并进行全面的青光眼检查，以确认诊断并预防视神经进一步受损。

Semi-Scleral Contact Lens Success Story

At Family Eyecare Optometry Centre, we recently managed a patient with bilateral keratoconus who struggled with very poor unaided vision.

👁️ Before (Unaided Vision):
Right Eye: Counting Fingers @ 30 cm
Left Eye: 3/60
The patient had significant blur and could not rely on spectacles due to the irregular corneal shape caused by keratoconus.

After Fitting Semi-Scleral Lenses (Best Vision):
Right Eye: 6/7.5+2
Left Eye: 6/7.5+2

Case Sharing from our Practice
Patient: 46-year-old female
History: No diabetes mellitus (DM) or hypertension (HPT)
Refractive status: High myopia
Intraocular Pressure (NCT): R 17 mmHg, L 17 mmHg

Findings:
Fundus photo shows a tilted optic disc with peripapillary atrophy, commonly seen in high myopia. A small flame-shaped haemorrhage is noted near the superior disc rim. The macula appears flat and healthy.

Comments:
Disc haemorrhage may be related to myopic changes or early glaucoma signs. Regular eye check, OCT, and visual field monitoring advised.

Plan:
Refer to Ophthalmologist for dilated eye examination and
OCT and visual field monitoring.

👁️ Take-Home Message
A small disc haemorrhage may be linked to high myopia or early glaucoma.
💡 See your optometrist for a detailed eye screening and regular follow-up.

屈光状况： 高度近视
眼压（非接触式）： 右 17 mmHg，左 17 mmHg

检查结果：
眼底照片显示视神经盘倾斜伴视盘周围萎缩（高度近视常见）。视盘上缘可见轻微出血点。黄斑区平整、反光正常。

备注
视盘出血可能与近视性改变或早期青光眼征象有关。建议定期进行眼科检查、OCT（光学相干断层扫描）及视野监测。

计划：
转介至眼科医生进行散瞳眼底检查，并安排OCT（光学相干断层扫描）及视野监测。

As the national body representing optometrists in Malaysia, AMO remains committed to advancing eye health through professional excellence, public education, and collaborative initiatives with governmental and private sectors. Preventable blindness and vision impairment continue to affect millions worldwide, and many of these conditions can be avoided with early detection and proper management.

Through our ongoing efforts in Primary Eye Care, community screenings, and public awareness programmes, AMO strives to ensure that every Malaysian has access to quality vision care services. We encourage everyone to take proactive steps to protect their sight—by scheduling regular eye examinations and adopting healthy visual habits.

On this World Sight Day, AMO calls upon individuals, healthcare providers, and policymakers to work together in building a future where no one is left behind due to avoidable vision loss.
