55 years old Male , known case of Chronic Kidney Disease (CKD)

Combo images showing small echogenic kidney – consistent with known CKD.

Note also presence of thin, hypoechoic, extracapsular fluid collections around the kidney. This is call ‘kidney sweat sign’.

This fluid is thought to represent perirenal oedema (Cecair sekeliling buah pinggang) seen in renal failure patients.

It can be appreciated on ultrasound, CT and MRI.

Differential diagnosis are perirenal hematoma & perirenal abscess

No obstruction/hydronephrosis is present

Ultrasound is not able to distinguish between partial and complete duplication.

Most often duplex system is unilateral but, in this case, it is bilateral.
