PEPTIC ULCER DISEASE
Ulcer simply refers to a wound that bruises easily but heals very slowly.when it occurs in the gastro intestinal tract we call it peptic ulcer.

Peptic ulcer disease occurs when the gastro -mucosal protective factors are overwhelmed by mucosal destructive factors.

Peptic ulceration commonly involves the stomach,duodenum and lower oesophagus.
If it occurs in the stomach,it is called GASTRIC ULCERS
If it occurs in the duodenum,it is called DUODENAL ULCER.

The healing of peptic ulcer can be promoted by stopping smoking and taking antacids and by anti secretory drug treatment.
However,relapse is common when treatment ceases...........

HYPOGLYCEMIA
Hypoglycemia simply means LOW BLOOD SUGAR LEVEL. Patients with long standing type 2 Diabetes as well as type 1 diabetic patients are prone to hypoglycemia.

What are the Symptoms and signs of Hypoglycemia?
1.Shaking and trembling.
2.sweating
3.pins and needles in lips and tongue.
4.Hunger.
5.palpitation
6.Headache
7.double vision
8.difficulty in concentration.
9.slurring of speech
10.confusion
11.change of behavior
12.convulsions
3 Unconsciousness.

How is Hypoglycemia managed?

Initially glucose 10--20g is given by mouth either in liquid form or as granulated sugar or sugar lumps.

Alternatively, 10g of glucose is available from non-diet versions of lucozade energy drink 55ml,coca-cola 100ml,Ribena blackcurrant 19ml.

If glucose cannot be given by mouth,if it is ineffective or if the hypoglycemia causes unconsciousness,glucagon 1mg(1 unit) should be given by intramuscular route or subcutaneously.
For a child 8 yrs and younger or with a body weight under 25kg should be given 500micrograms of glucagon..

SYNCOPE
Sycope is another word to describe the condition called "fainting" or loss of consciousness.
Insufficient blood supply to the brain results in syncope,the commonest cause is a vasovagal attack or simple faint due to emotional stress.
Other causes are:
#1.postural hypo tension which can be a consequence of rising abruptly or of standing upright for too long.
2 #.antihypertensive drugs can predispose to syncope
3 #.under stressful conditions, some patients hyperventilate which gives rise to feelings of faintness.
Symptoms and Signs
*patient feels faint
*low blood pressure
*pallor and sweating
*yawning and slow pulse
*nausea and vomiting
*dilated pupils
*Muscular twitching.
How is syncope Managed?
First,
Lay the patient as flat as is reasonably comfortable and in the absence of associated breathlessness,raise the legs to improve cerebral circulation.
Second,
Loosen any tight clothing around the neck.
Lastly,
Once consciousness is regained,give sugar in water or a cup of sweet tea...

Asthma is a disease of airway inflammation,which results in constriction of the bronchioles.
Signs and symptoms include:
*Breathlessness
*wheezing
*chest tightness

Asthma can be diagnosed by history,physical examination and pulmonary function tests.

How is Asthma treated?
Treatment involves controlling triggering factors and drug therapy most commonly with inhaled corticosteriods and a class of drugs Called beta agonist.

*CEREBRAL MALARIA*
What is cerebral malaria?

It is a severe case of malaria where the brain has become involved, often manifesting as if a psychiatric disorder.

What type of plasmodium causes cerebral malaria?

Mosquito bites transmit at least any of the four types of plasmodium,namely:
*P.vivax
*p.ovale
*p.malariae and
* p. Falciparum.
of these four,plasmodium falciparum is notorious for causing *cerebral malaria*

How does p.falciparum cause cerebral malaria?

It does this by infecting red blood cells (RBC's),when it does,it makes them sticky,the sticky RBC's stick to one another and to the walls of the brain capillaries. This leads to "traffic jam" in the tin y capillaries of the brain,leading to the classical symptoms of cerebral malaria.

What are the Classical signs and symptoms?

*migraine-like headache
*very high fever
*chills
*convulsions
*in untreated and severe cases...coma a.n.d death.

How is cerebral malaria treated?

Parenteral antimalarials are given,followed by tablet antimalarials... Then the symptoms are managed with analgesics and other drugs your doctor or pharmacist may deem fit.
