*Drug Interaction between Fexofenadine and Rifampicin*

Fexofenadine is an antihistamine (selective H1-receptor blocker) used in the treatment of allergies like nasal congestion, sneezing, itching of eyes, urticaria, etc. It is a second generation antihistamine with lesser tendency of crossing the blood brain barrier, hence, lesser tendency of sedation.
Rifampicin is a Rifamycins broad spectrum antibacterial agent used in treating bacterial infections including tuberculosis.

The concomitant use of both Fexofenadine and Rifampicin would lead to a moderate drug interaction. This is because Rifampicin induces intestinal P-glycoprotein, causing increased secretion of Fexofenadine which consequently leads to decreased absorption of Fexofenadine. Studies have revealed that there is increased oral clearance of Fexofenadine when both drugs are coadministered, also, peak plasma concentrations dropped by 32%. Hence, sub-therapeutic plasma levels of Fexofenadine would lead to poor allergy control.

This is considered a moderate drug interaction in which an alternative is advised in other to achieve proper allergy control by Fexofenadine.

***Drug Interaction between Digoxin and Azithromycin***

Digoxin is a cardiac glycosides used for the management of congestive heart failure and atrial fibrillation amongst other arrhythmias. Azithromycin is a macrolide antibiotic used in treating bacterial infections.

The concomitant use of both Digoxin and Azithromycin can lead to a major drug interaction in which the patient is susceptible to high plasma levels of Digoxin. Two possible mechanisms have been proposed. Generally, macrolide antibiotics have been known to deplete intestinal load of _Eubacterium lentum_ which is partly responsible for the metabolism of Digoxin. Hereby, leading to high Digoxin absorption levels. Also, macrolides are major inhibitors of P-glycoprotein, of which Digoxin is a substrate. This leads to decreased tubular secretion of Digoxin in the kidneys. Hence, both mechanisms leads to further increase in the plasma levels of Digoxin.

This is considered a major drug interaction in which close monitoring of Digoxin plasma levels or an antibiotic alternative is advised.

Other examples of macrolide antibiotics include Clarithromycin, Erythromycin, etc.
