Community health practice refers to provision of health care services, aimed at early diagnosis of disease, recognition of environmental and occupational hazards to good health and prevention of disease in the community. The Community is seen as the hub of Community Health Practice. It is the essential laboratory for practice of teaching, training and research in the subject of community medicine/health. The idea of the community as the centre of health services delivery was advocated as far back in 1960s. From the concept of Basic Health Services, Primary Health care emerged. In this regard, the principle of health services in relation to availability, accessibility, acceptability and appropriateness became important considerations in WHO health policy from the late 1960s and into the 1970s. The goal of Primary Health Care (PHC) was to provide accessible health for all by the year 2000 and beyond. Unfortunately, this is yet to be achieved in Nigeria and seems to be unrealistic in the next decade if the basic essence of the Universal Health Coverage – making health care accessible, available and affordable to rural poor is ignored to whatever extent. This work seeks to examine some cross cutting issues in community health practice in Nigeria and to outline triggers for community oriented health manpower in Nigerian health system and some prospects in the midst of obvious challenges. Essentially, community health practice using the policy and principles of Primary Health Care in Nigeria, especially in rural communities have come a long way and still require more to be done to achieve the goal for health for all now and beyond... What’s hepatomegaly?

Hepatomegaly is having an enlarged liver. Your liver is the largest internal organ. It helps your body:

digest fats

store sugar in the form of glycogen

fight off infections

produce proteins and hormones

control blood clotting

break down medications and toxins

The liver is also the only internal organ that can grow back after surgery, which makes live liver donation possible. If you donate a portion of your liver, it will regenerate to its original size. The transplanted portion will also grow.

If you have an enlarged liver, it could mean that you have:

a liver disease

cancer, such as leukemia

a genetic disease

heart and blood vessel abnormalities

an infection

toxin poisoning

Many of the symptoms that cause hepatomegaly can impair your liver’s ability to function and help your body.

While hepatomegaly is always a cause for medical evaluation, not all of the underlying conditions are considered medical emergencies. Make an appointment with your doctor if you have signs or symptoms of an enlarged liver.

What are the signs and symptoms of hepatomegaly?

An enlarged liver on its own may not have any symptoms. But if a medical condition is causing your enlarged liver, you may experience serious symptoms such as:

jaundice, or yellowing of the skin and eyes

muscle aches

fatigue

itching

nausea

vomiting

abdominal pain or mass

poor appetite

swelling of the feet and legs

easy bruising

weight loss

increasing abdominal size

Any of these symptoms require evaluation by your doctor.

Call 911 or seek emergency medical attention immediately if you have:

severe abdominal pain

fever and jaundice

bloody or coffee ground vomit

shortness of breath

INTRODUCTION
Many people use to says we are not smoking, while perhaps they are not but they are always staying together with the people who are smoking. So in this case they to be call secondhand smokers, meaning they are also known as smokers but how and why????
What is secondhand smoke?
Secondhand smoke is the combination of smoke from a burning cigarette and smoke exhaled by a smoker. There are two types of secondhand smoke; side stream smoke comes directly from the burning to***co product, and mainstream smoke is the smoke that the smoker inhales. The smoke that burns off the end of a cigarette or cigar actually contains more harmful substances than the smoke inhaled by the smoker, as there is no filter it must pass through.
How does secondhand smoke affect non-smokers?
If you are a non-smoker but are exposed to secondhand smoke on a regular basis, your body will still absorb ni****ne and other harmful substances. Smoke contains more than 4000 chemical compounds, of which 250 are toxic and more than 50 are known cancer-causing agents. These dangerous substances linger in the air for approximately
4 hours and breathing in these particles for only minutes can harm you.
Exposure time to secondhand smoke and effects:
5 minutes – stiffens the aorta as much as smoking a cigarette
20-30 minutes – causes excess blood clotting, as well as increases the buildup of fat deposits in blood vessels, increasing the risk of heart attack and stroke.2 hours – increases the chance of irregular heart beat (arrhythmia) and can trigger a fatal cardiac event or heart attack.
In addition, the longer you are around secondhand smoke, the greater the level of harmful substances in your body. As a result, you might have an increased risk of developing smoking-related disorders, including:
Lung cancer and lung disease, including COPD, emphysema, asthma, and hea

STATISTICAL EVIDENCED ON THE CASE OF SEXUAL VIOLENCE...Studies from different parts of the world have found that 7-36% of girls and 3-29% of boys suffer from s*xual abuse in childhood, with a majority of studies reporting 1.5-3 times more s*xual violence against girls than boys. MY THINKING ABOUT RISK AND VULNERABILITY
Few people are able simply to accept information about what is good for them and make the necessary changes in their lives . You should be aware of situations and behaviour that influence STI risk and vulnerability, and take a realistic approach to behaviour change. Risk and vulnerability are influenced by behaviour as well as by other factors,such as age and gender,the place where one live works, and the larger social,cultural and economic environment, which may be beyond the person power to change. Migrant workers who are separated from their families for along periods of time may have risky s*x because they are lonely; poor people often have poor access to health care services; and some women and men are forced to sell or trade s*x in order to survive or support their families..
