How to record in care to avoid problems

In care, what you write is very important. It can protect you and the person you support. Always write what you see, hear, or do. Do not write what you think or feel.

For example, you checked a resident in their room and they are lying down and to you they are sleeping because their eyes are closed. Please write appeared to be asleep not sleeping. It maybe they are no longer alive but appear sleepy! If you want to sound so acurate: you can tap them a little or stand by them for few seconds and observe they are breathing in and out.

Write declined personal care I will try again later not refused personal care.

Write raised voice and waved hands while speaking not aggressive. Saying agressive can cause you to explain more.

Write asked where she was several times not confused.
Write only what you observe and not what you feel because good recording can save you from lots of trouble.
