Understanding Vaginitisđ§đŠşđ
Vaginitis is an inflammation or infection of the va**na, often causing discharge, itching, irritation, or odor. It can result from infection, imbalance in va**nal flora, or irritation from chemicals or allergens.
đMain Causes & Types
1. Bacterial Vaginosis (BV)
â˘Overgrowth of Gardnerella va**nalis and anaerobic bacteria
â˘Thin, gray-white discharge, fishy odor (especially after s*x), va**nal pH >4.5, clue cells on wet mount
2. Candidiasis (Yeast Infection)
â˘Candida albicans
â˘Thick, white âcottage cheeseâ discharge, intense itching, redness, pH usually normal (â¤4.5), budding yeast/hyphae on KOH prep
3. Trichomoniasis
â˘Trichomonas va**nalis (protozoan parasite)
â˘Frothy yellow-green discharge, foul odor, strawberry cervix, pH >4.5, motile trichomonads seen on wet mount
4. Atrophic Vaginitis
â˘Low estrogen levels (postmenopause, lactation)
â˘Dryness, irritation, dyspareunia, thin mucosa, pH >4.5
5. Noninfectious/Allergic Vaginitis
â˘Soaps, detergents, sprays, condoms, etc.
â˘Burning, irritation, minimal discharge, no infection on test
đDiagnosis
⢠History & Physical Exam
⢠Vaginal pH test
⢠Normal: 3.8â4.5
⢠BV or Trichomoniasis: >4.5
⢠Wet mount / KOH prep
⢠Clue cells â BV
⢠Hyphae/yeast â Candida
⢠Motile trichomonads â Trichomoniasis
⢠Whiff test: Fishy odor after adding KOH â BV positive
⢠Culture or NAAT (for confirmation if needed)
Noninfectious
â˘Avoid irritants; use mild soaps, topical hydrocortisone if needed
đPatient Education
⢠Avoid douching or scented feminine products
⢠Wear breathable cotton underwear
⢠Complete prescribed medications even if symptoms improve
⢠Encourage partner treatment for Trichomoniasis
