What is hydronephrosis?

Hydronephrosis describes the situation where the urine collecting system of the kidney is dilated. While obstruction or blockage is the most frequent cause of hydronephrosis, it may be due to problems that occur conge***ally in a fetus (prenatal) or may be a physiologic response to pregnancy.

Technically, hydronephrosis specifically describes dilation and swelling of the kidney, while the term hydroureter is used to describe swelling of the ureter. Hydronephrosis may be unilateral involving just one kidney or bilateral involving both.

What causes hydronephrosis?

There are numerous causes of hydronephrosis that are categorized based upon the location of the swelling and whether the cause is intrinsic, extrinsic or if it due to an alteration in function. Colicky pain comes and goes and its intensity may cause the person to writhe or roll around or pace in pain. There may be blood seen in the urine.

Chronic hydronephrosis develops over time and there may be no specific symptoms. Tumors in the pelvis or bladder obstruction may develop silently and the person may have symptoms of kidney failure. These are often nonspecific and may include weakness, malaise, chest pain, shortness of breath, leg swelling, nausea and vomiting. If electrolyte abnormalities occur because the kidneys are unable to regulate sodium, potassium, and calcium, there may be heart rhythm disturbances and muscle spasms.

FIBROMYALGIA

Fibromyalgia is a chronic pain condition; it commonly affects the muscles and ligaments and usually has been present for years when a physician diagnoses the condition. Fibromyalgia was formerly known as fibrositis.

CAUSES

Fibromyalgia has been shown to be genetic. It frequently becomes evident after stressful events. The stressful events may be emotional (such as a traumatic life event), physical (such as a motor-vehicle accident), or medical (such as certain infections).

The chronic pain of rheumatoid arthritis, systemic lupus erythematosus, and other autoimmune diseases can trigger the development of fibromyalgia.The manner in which the brain and spinal cord process pain sensations is abnormal in fibromyalgia.

The threshold at which stimuli cause pain or discomfort has been proven to be lower in fibromyalgia. The pain felt is more intense because the pain is amplified by the abnormalities in the central nervous system and in pain processing. In addition, fibromyalgia causes the pain from any given cause to be worse.

SYMPTOMS AND SIGNS

The defining feature of fibromyalgia is chronic widespread pain. This means pain in multiple areas of the body, most commonly in muscles, tendons, and joints.

The pain is generally above and below the waist, on the left side of the body and on the right side of the body but can be localized, often in the neck and shoulders or low back, initially.
The pain is chronic, which means it is present for more than three months. Fatigue is the other universal symptom of fibromyalgia. Patients commonly feel they sleep "lightly" and may have multiple night-time awakenings with difficulty returning to sleep.

Fibromyalgia is associated with many other symptoms. Disordered thinking (cognitive disturbances) is often referred to as "fibro fog." Depression and anxiety are present in 30%-50% of patients at the time of diagnosis with fibromyalgia.

Headaches are present in more than half of patients. Patients also may have a variety of poorly understood pain symptoms, including abdominal pain, dry eyes, dry mouth, chest wall pain, pelvic pain, and bladder symptoms, heart palpitations, numbness and tingling, multiple allergies and chemical sensitivities, weight gain, and others.

The physical examination is remarkable for tenderness in specific anatomic locations, such as the back of the neck where the neck muscles connect to the skull. There are 18 such locations, which are referred to as fibromyalgia tender points.

What is bullous pemphigoid?

Bullous pemphigoid is an uncommon skin disease characterized by tense blisters on the surface of the skin. Occasionally, the inner lining tissue of the mouth, nasal passages, or conjunctivae of the eye can be involved.

The condition is caused by antibodies and inflammation abnormally accumulating in a particular layer of the skin or mucous membranes. The mucous membrane disease is also referred to separately as mucous membrane pemphigoid.

A majority of those affected by bullous pemphigoid are 50 years of age or older. While the cause is unknown, it is felt by some that an aging immune system may become activated in certain individuals with a genetic predisposition to develop bullous pemphigoid.

What are symptoms of bullous pemphigoid?

Symptoms of bullous pemphigoid include intense itching and burning sensation of the skin. When the mucous membranes of the mouth are affected, it can cause pain, burning, peeling away of affected inner lining tissues, and sensitivity to acidic foods.

Eating can be difficult, and involvement in the deeper areas of the throat can cause coughing. Involvement of the inner nose can cause nosebleeds. The disease typically worsens and improves over time.

Bulimia, also called bulimia nervosa, is one of a number of eating disorders. This mental illness is characterized by episodes of bingeing and somehow purging the food and/or associated calories in the pursuit of weight loss.

What are causes and risk factors for bulimia?

While there is no known specific cause for bulimia, family history and environmental stressors are thought to contribute to the development of the illness.

Adolescents are at greatest risk for developing bulimia, Adolescents who have any eating problem by 12 years of age are at higher risk for developing bulimia.

What are symptoms and signs of bulimia?

Symptoms of bulimia include repeated episodes of bingeing and purging. Binges are defined as uncontrolled episodes of eating food amounts in a short period of time that are clearly more than most people would consume in a similar time period. People with bulimia have trouble controlling their eating during the binges.

They also engage in some form of repeated undoing of the excessive food/calories they ingest that occurs during episodes of binge eating. Examples of purging behaviors include making oneself throw up, fasting, excessive exercise, or abuse of laxatives, diet pills, diuretics, or other medications. Individuals with bulimia may develop swollen salivary glands that give the sufferer bigger cheeks. They may also develop constipation, dehydration, dry skin, and thinning hair.
