1) Primary infertility
A couple has never achieved pregnancy before.
Common causes
Ovulation problems (PCOS, hormonal imbalance)
Blocked fallopian tubes (often from past infection)

Treatment approach (usually more investigative first)
Doctors normally start with full evaluation:
Hormone profile (FSH, LH, prolactin, thyroid)
Ovulation tracking
HSG test (tube test)
Semen analysis
Pelvic scan
Treatment depends on findings
Ovulation induction drugs
Hormone correction
Tube opening surgery / IVF if tubes blocked

2) Secondary infertility
A couple conceived before but cannot conceive again.

Different from primary — usually acquired problems:
Pelvic infection after delivery or abortion
Scarred or partially blocked tubes

Endometritis (very common but overlooked)
Reduced egg quality due to age
New s***m issues
Treatment approach (more targeted)
Doctors focus on what changed after last pregnancy

Treat infection/inflammation

Manage fibroids or adhesions
Mild ovulation stimulation
Secondary infertility often responds faster because:
The reproductive system has proven it can conceive before — the problem is usually damage or imbalance, not absence of function.
