Mooi om meegewerkt te hebben aan zo'n belangrijk artikel!
Hopelijk luidt dit het einde in van de volledig opgebakken kroon, een onnodig zwakke constructie, welke chipping in de hand werkt.
Dit artikel gaat over indirecte restauraties in het front, op natuurlijke elementen en implantaten, maar het principe geldt ook voor indirecte restauraties in de zijdelingse delen,
waar mogelijk monolytisch, alleen de buccale vlakken opgebakken porselein.

The 54 year young woman with this lovely smile showed up in our practice 19 years ago with a discolored left central incisor ( fig 2) after being accidentally hit by a schoolmate during play when she was 8 years old. The tooth reacted negative on sensibility tests but showed no signs of apical reaction. Her chief complaint is the darker color of the tooth. What is the right way to go?
We know that there is a risk of further obliteration of the root canal and a possibility for a need of RCT when complaints occur later. Can we do a proper RCT then? Or is it better to do the RCT right away in combination with internal bleaching ( what is the benefit, are there any risks?)
That’s where the daughter-test comes into play: “what would you do if it was your daughter..?” We decided to delay the risk and simply bleach the tooth from the outside. Fig 2 also shows the result after 4 months of bleaching (daily) with 16 % carbamide peroxide and fig 1, 3 and 4 show the 19 year follow up. Still no signs apically, a steady result of the home bleaching and a happy patient. Could we have had the same result with performing RCT and internal bleaching? Probably, but what would it mean for the prognosis on the long term? I’m not sure, but like Bertrand Russell said once: “ Not to be absolutely certain is, I think, one of the essential things in rationality.”

If a person shows more than 2 mm of gingiva when smiling and also the smiline is not horizontal, this can give discontent from an esthetic point of view.
If the cause of the gummy smile is overgrown gingiva during growth failing to recede to the normal apical level (altered passive eruption), this can easily be corrected by periodontal surgery.

In the esthetic zone after extraction of a lost tooth it is important to "maintain" hard and soft tissue, rather than trying to "regain" it after time.
If there is an intact buccal plate and medium to high lipline it is our first choice procedure.
