As we age, the face doesn’t just “wrinkle.”
There are predictable anatomical changes happening underneath the skin:
1) Bone resorption :the facial skeleton slowly remodels and loses projection, particularly in the midface, jawline and chin.
2) Fat pad descent: supportive fat compartments shift downward due to ligament laxity and gravity.
3) Ligament weakening: retaining ligaments that once held tissues in place become less supportive.
4) Collagen and elastin decline: skin becomes thinner, less elastic and more prone to creasing.
5) Muscle activity: repeated movement creates etched lines over time.
This combination leads to what we see externally like nasolabial folds,marionette lines, loss of cheek support, jowling or heaviness, flattening of facial contours which creates a more tired or drawn appearance.
Dermal filler, when used correctly, is not about “filling lines”. It is about replacing structural support where it has been lost.
