So eto nga...

Nawala ang wallet ko na walang laman pera pero yung lahat ng importante kong ID's ay naruon nuong January 14, 2026 sa aking workplace.

Ngayon sumasakit na ang ulo ko dahil I need to file police report pa daw para makakuha ako ng Affidavit of Loss kung ang reason ko e NINAKAW yung aking wallet.

Kung sinu ka man. Nakikiusap ako sa iyo na pakisauli lang yung mga ID's ko. Huwag mo naman itapon kung saan😭 Napahirap magre apply ng mga ID's sa totoo lang. Napakalaking abala ang ibibigay MO sa akin.

Maawa ka naman. Please lang🙏🙏🙏🙏

What: Medical and Dental Examination
When: JANUARY 13-14, 2026
Time: 9:00 AM to 2:30PM AS SCHEDULED

Where: URS Angono Room 107

REMINDERS:
1. Please be on time.
2. Procedure/Steps:

- STEP 1: Kumuha ng number para sa pagkakasunud sunod
Pakikuha po ang inyong Medical/Dental Form kay nurse bago kayo pumila. Kung may abnormal findings at may instructions ni nurse, pakibigay kay nurse medical certificate o copy ng reseta(kung meron) para mai-attach sa inyong form
- STEP 2: Pumunta sa may lababo (sink) para mag gargle ng Mouth wash solution sa loob ng isang minuto.
- STEP 3: Pumila sa Medical (Table 1) o Dental (Table 2) para sa inyong evaluation.
- STEP 4: Kapag tapos na, ibalik ang Medical/Dental Form kay Nurse. Pumirma sa monitoring list. Isauli ang inyong number bago kayo umalis.

3. Kung kayo ay magkakaroon ng ubo o sipon ay mangyari po lamang na kumunsulta na agad sa pinakamalapit na Health Center o Municipal Health Office.

For your information and guidance. Kung kayo po ay may tanong pakimessage nyo na lang po si Nurse. Maraming salamat po.

And Para po sa mga nagtatanong:

1. Kung kayo po ay female at nakapagpalaboratory sa campus last December 26, 2025 hindi na po ninyo NEED MAGDALA NG PREGNANCY TEST KIT during Initial Assessment.

2. Kung kayo po ay walang health condition o abnormal findings sa results ng inyong lab tests, Hindi nyo po need kumuha ng medical certificate sa labas. Ang tinutukoy ko pong Medical certificate ay yung FORM na galing sa ating university. Sa akin (Kay nurse) nyo po ito kukunin during Initial Assessment.

3. Sa Medical and Dental Examination schedule po on January 13 at 14, 2026 ay university physician at dentist na po makakaharap ninyo. Dito na po I evaluate if you are FIT FOR OJT.

4. Please wag nyo po kakalimutang I photocopy Ang result ng inyong lab tests. Isang copy lang po.

5. Sundin po natin schedule ng Initial Assessment at Medical and Dental Examination. First Come, First Serve basis po ito.

1. Attached is the letter of Medical Dental Exam. Kindly read😁.

2. Pregnancy Test (for female only - the test will be done at the URS Angono Clinic, please bring your own Pregnancy Test Kit)‼️

3. The schedule for the Initial Assessment of your Medical requirements is on January 6-8, 2026 9AM to 3PM. Please bring Original and PHOTOCOPY of your results. ‼️

4. Our University Physician and Dentist will be present on January 13 and 14, 2026 for your medical examination and evaluation.

5. Should you have inquiries or clarifications kindly message your Campus Nurse‼️ (Monday - Thursday 8AM to 4PM)
