Hematoma After Phlebotomy: What Laboratory Professionals Should Know

What is a hematoma?
Localized collection of blood outside the vein due to vessel injury during venipuncture.

Common causes:
• Vein punctured through-and-through
• Inadequate pressure after needle removal
• Fragile veins (elderly, pediatric, dehydrated patients)
• Large-bore needle or improper angle
• Anticoagulant therapy (e.g., aspirin, warfarin)

Early signs:
• Swelling at puncture site
• Bruising or discoloration
• Pain or tightness
• Site enlarges over hours to days if bleeding continues

Ongoing care:
• Cold compress (first 24 hours) → reduces bleeding
• Warm compress (after 24–48 hours) → promotes reabsorption
• Avoid heavy lifting or repeated venipuncture at the site

When to seek medical attention?
• Rapidly increasing size
• Severe pain or numbness
• Signs of infection (redness, warmth, fever)
• No improvement after several days
