REMINDERS:
1. For representative of deceased beneficiary, kindly bring 2 copies of death certificates of bene
2. For authorized representative of beneficiary, provide printed copy of picture of bene holding the DECEMBER ,2025 calendar together with the representative holding the authorization form (magkasama/ magkatabi sa picture si senior at bene). Don't forget the 3 signatures/ thumbark of senior on ID, and 1 signature in authorization form.
3. In xerox/ photocopy of Senior's ID and Representative ID, kindly put active cell phone numbers (make sure may nakalagay na before lumapit sa paymaster/s).
4. Wag lulukutin or tutupiin ang mga papel/ requirements
5. BE ON TIME, as much as possible come early before the scheduled time (para may time pa silang ma-arrange alphabetically or according sa kanilang payroll number)
