-HD Prescription/Referral from Nephrologist
-Medical Abstract (from previous dialysis center)
-3 Latest Treatment Sheets (from previous dialysis center)
-Latest Lab Results (CBC/PC, Blood Chem and Xray Result) with in 1 month and blood typing.
-Latest Hepatitis Profile (HBsAg, Anti-Hbs, Anti-HCV, Anti HBc total with in 6 months.
-Latest copy of maintenance Medications.
Philhealth Requirements:
-Updated Members Data/MDR
-For self-paying: Latest 3 months of contribution/SOA
-PhilHealth Availment/Consumption (from previous dialysis center)
-Patient Dialysis Database (PDD) from your previous dialysis center.
-Other Pertinent medical records such as ultrasound,ECG's,X-ray,etc.
-Covid19-Antigen Test.
-Guarantee Letter from Cabiao MSWDO for Cabiao residence.

Alam niyo ba na may link sa pamamagitan ng problema sa bato at mataas na *triglycerides. Dagdag mo pa ang di kanais-nais na lab results sa LDL at HDL, ang triglycerides ang isa sa sanhi ng lumalalang sakit sa bato.

Madalas nating marinig na masama ang taba para sa kalusugan ng ating puso. Bukod dun, kapag sumobra sa taba, maaaring hindi sapat ang daloy ng dugo para sa bato. Bawas-bawasan natin ang taba sa ating pagkain.
