Attention to ALL enrolling freshmen, returnees, transferees and shiftees of BISU - Calape Campus!
Medical Requirements for the 1st Semester Enrollment of A.Y. 2025-2026:
General Requirement for All Programs:
1. Medical Certificate (must indicate Physically Fit for School/Enrollment)
2. Chest X-ray (result, not the film)
3. COVID-19 Vaccination Card (for vaccinated only)
4. Pre-Enrollment Health Form (see link below)
Additional Requirements per Program:
BS Midwifery:
1. Complete Blood Count (CBC)
2. Urinalysis (U/A)
3. HBsAg (Hep B test)
4. Neuro-Psycho
BSIndTech FPST:
1. Stool Exam
2. Health Card
3. HBsAg (Hep B test)
BSIndTech ElectTech:
1. Ishihara Test (Color Blind Test)
BSCS/BS Fisheries/BSEd-English&Math/BEEd:
-General requirements only
INSTRUCTIONS:
1. Submit readable photocopies only using A4 sized bond paper.
2. All medical requirements may be acquired from any PUBLIC or PRIVATE physician, clinic or laboratory.
3. Pregnant enrollees are NOT required to submit Chest X-rays but will be required to present photocopy of ultrasound (if available only) and Prenatal Record.
4. Photocopy of COVID-19 Vaccination Card (printed picture if vaccination card is lost).
5. Bring photocopies of old medical records from previous medical conditions /illnesses (if available only)
6. Submit medical requirements in a long hard brown envelope.
7. Questions and clarifications may be sent to this clinic page.
8. Incomplete requirements will not be accepted.
Note:
1. Students enrolled during the 2nd Semester of A.Y. 2024-2025 are considered CONTINUING students and are not required to submit the requirements listed above. (except incoming 4th Year OJT enrollees)
2. For 4th Year OJT enrollees, please see list of requirements below.

The Campus Health Services will be facilitating a FREE Dental Extraction and Consultation on May 7, 2025 at the Campus Clinic. The activity is open to all students, faculty & staff.

2. First Come, First Served protocol is observed during extraction
3. The University Dentist can only cater up to 20 patients per visit
