Requirements for Delayed (Late) Registration;
✔ Original Certificate of Negative Record (INTACT) from PSA and Form 1B from CCRO
(REQUIRED for 3 years old & above)
(VALIDITY: 6 months only from date of issuance)
✔Certificate of Live Birth (COLB) Form (4 copies)
(All entries must be typewritten with no erasures and readable)
(With signature of attendant at birth) - Traditional Birth Attendant or Hilot😊
✔Valid ID of Traditional Birth Attendant or Hilot with picture and signature
(For birth that occurred outside the hospital/maternity/lying-in clinic)
✔Accomplish Affidavit for Delayed Registration at the back of COLB Form
✔Photocopy of valid identification card (ID) of both parents/applicant with signature (plus three (3) fresh signatures)
✔Original Certificate of Marriage from PSA - plus one (1) photocopy (xerox), or certified copy (parents/applicant)
✔Affidavit of Two Disinterested Persons who might have witnessed or known the birth of the child
(Attach photocopy of valid IDs with picture and signature of the two witnesses)
✔Authorization Letter from parents/applicant
(Authorized person must present valid ID with picture & signature - plus one (1) photocopy (xerox)
SUBMIT ORIGINAL OR CERTIFIED COPY OF ANY TWO (2) OF THE FOLLOWING SHOWING THE NAME OF THE CHILD, DATE AND PLACE OF BIRTH, AND NAME OF MOTHER/FATHER
✔Baptismal Certificate/Dedication Certificate
✔School Record - Form 137/Transcript of Records
✔Income Tax Return of parent/s
✔Insurance policy
✔Pre-natal Record of Mother/Immunization Card of the child
✔Others:
NBI Clearance/Police Clearance,
Barangay Captain's Certification,
SSS/GSIS/Philhealth MDR/Pag-ibig,
Employment Record,
Voter's Certificate/Voter's Affidavit (COMELEC),
Community Tax Certificate (CEDULA)
ADDITIONAL REQUIREMENTS
(FOR ILLEGITIMATE CHILD)
✔Born before 03 August, 1988, the FATHER must sign the Affidavit of Acknowledgement at the back of the COLB
✔If FATHER is DECEASED, submit documents that will prove filiation of the child
✔Born from 03 August, 1988 to 18 March 2004, with or without Admission of Paternity (AAP) at the back of the COLB, the child shall use the SURNAME OF THE MOTHER
*BORN FROM 19 MARCH, 2004 TO PRESENT;
✔With AAP but no Affidavit to Use the Surname of the Father (AUSF), the child shall use the SURNAME OF THE MOTHER
✔With AAP and with AUSF, the child shall use the SURNAME OF THE FATHER
