Because these informations are important for the doctor to make an adequate and accurate diagnosis. We don't treat patient based on symptoms alone. The human body and physiology is intricate and disease process is complicated often the set of symptoms are associated with something in your medical history. A thorough investigation is necessary to identify and address which disease process is ailing you. These are records that are kept and will aid our doctor if you return for consult in some other days and for continuation of management.
