📢 PUBLIC ADVISORY: PhilHealth YAKAP Program Registration Update

In line with the latest advisory from PhilHealth, all beneficiaries are reminded that self-registration is required to select your preferred YAKAP (Primary Care) provider.

✅ How to Choose Your YAKAP Provider:

Walk-in beneficiaries must complete their selection through any of the following official platforms:

• 🌐 PhilHealth Member Portal
• 📱 eGov Super App

Selection must be done in real time via the digital platforms.

🔎 Important Reminder:

Primary Care (PC) Clinics may assist patients by providing equipment such as laptops or tablets to facilitate digital onboarding and YAKAP provider selection. However, the actual registration and provider selection must be completed by the member through the system.

This process ensures accurate enrollment and proper assignment to your chosen YAKAP provider.

For guidance, visit the official PhilHealth website or access the PhilHealth Member Portal / eGov App today.

👤 FOR MEMBER-PATIENTS:
✔️ PhilHealth ID
✔️ If not available, please present any valid government-issued ID for verification purposes

👨‍👩‍👧 FOR DEPENDENT MEMBERS:
✔️ Member Data Record (MDR) – for easy verification and tracing of the dependent’s PhilHealth number
OR
✔️ PhilHealth Number (if available) – to facilitate faster processing
