Requirements for 5-11 years old:
- PSA Copy of Birth Certificate OR Valid Student ID of the child
- Valid ID of Parent/Guardian
- DQR of the child

Requirements for 12 to 17 years old:
- Birth Certificate OR Baptismal Certificate of Minor
- Valid ID of Minor AND Valid ID of Parent/Guardian
- If accompanied by a Guardian: Barangay Certificate or Special Power of Attorney (SPA) of guardianship
- If minor with comorbidities: Bring Medical Clearance from Pediatrician
- For 2nd dose: Bring your Vaccination Card

Requirements for Adults:
- For 1st Dose: Bring a Valid ID
- For 2nd dose and Booster: Bring your ID and Vaccination Card

Update on Omicron Subvariants
