Chronic ectopic pregnancy is a rare and life threatening condition that is difficult to diagnose. It is a form of pregnancy wherein the fertilized egg implants outside the uterus (bahay bata) usually within the fallopian tube.
Small but recurrent bleeding leads to a haematocele formation which causes inflammatory reaction and formation of chronic pelvic adhesions (pagdikit dikit nito sa bituka, matres, at obaryo etc) resembling a complex pelvic mass.

Here we are presented a case of an 18yo, amenorrheic for 13weeks, pregnancy test positive, with occasional abdominal (hypogastric) pain and minimal vaginal bleeding. With a high index of suspicion plus an ultrasound result of tc ectopic pregnancy, patient was managed as a case of chronic ectopic pregnancy.

The patient underwent exploratory laparotomy. Upon laparotomy, approximately 200cc of hemoperitoneum was evacuated. there was an evidence of chronic inflammation with extensive omentum and large bowel adhesions (pagdikit ng fallopian tube sa bituka) surroundjng the left adnexal mass. Adhesiolysis (pagtanggal ng mga dikit dikit na bituka at fallopian tube) and left salpingectomy (removal of the entire left fallopian tube) was done. The patient tolerated the procedure well.

Always a good day to save lives! 🙏
