• REGISTRATION FEE – Php150.00 every anniversary date.
• PLANS – Monthly (Php400.00), Quarterly (Php1,200.00), Semi-annual (Php2,400.00), Annual (Php4,800.00).
• HOUSEHOLD COVERED – One (1) plan holder and five (5) beneficiaries.
• CONTESTABILITY – One (1) month (accident), three (3) months (natural death) and six (6) months (60 years and above).
• DEATH CLAIM REQUIREMENT – Death Certificate or Sealed Barangay Certificate.
• ELIGIBILITY – Must beyond contestability period, account should be active (not delayed), must provide death claim requirement.
• MEMBER BENEFIT – Php20,000.00 Cash Assistance.
NOTE: Php25,000.00 funeral service and hospital assistance will be available in three (3) years of the company.
• GRACE PERIOD – three (3) days after the billing period.
• LATE PAYMENT FEE – Php100.00 per month.
• PLAN CANCELLATION – After six (6) months of no payment.
• DELAYED ACCOUNTS – not covered/eligible for claims.
• DELAYED ACCOUNT REINSTATEMENT – After 48 hours (2 days) based on official receipt (no claims are qualified within 48 hours).
• All transactions should be made during office hours (Monday – Friday from 8:00am to 5:00pm). Beyond cut-off will be recorded/posted the following day.
